Antimicrobial resistance in urosepsis: outcomes from the multinational, multicenter global prevalence of infections in urology (GPIU) study 2003–2013

Abstract: Objective Primary objective was to identify the (1) relationship of clinical severity of urosepsis with the pathogen spectrum and resistance and (2) appropriateness of using the pathogen spectrum and resistance rates of health-care-associated urinary tract infections (HAUTI) as representative of urosepsis. The secondary objective was to provide an overview of the pathogens and their resistance profile in patients with urosepsis.Population and Methods A point prevalence study carried out in 70 countries (2003–2… Show more

“…2,3,15 The results of the present study should be read in the light of the continuing increase of resistant bacterial strains and the necessity to find new strategies to reduce indiscriminate use of antibiotics, especially in patients where treatment is not indicated. 18,19 The present results suggest that an increase in the mean number of leukocytes of >150% from baseline might be considered a diagnostic aid for detecting the transition to symptomatic UTI. Finding an increase in the number of leukocytes justifies the need for a urine culture and tailored antibiotic treatment of patients with rUTI.…”

“…Differentiating ABU from symptomatic UTI is particularly important, because treatment of ABU will do more harm than good, as stated by several authors . The results of the present study should be read in the light of the continuing increase of resistant bacterial strains and the necessity to find new strategies to reduce indiscriminate use of antibiotics, especially in patients where treatment is not indicated . The present results suggest that an increase in the mean number of leukocytes of >150% from baseline might be considered a diagnostic aid for detecting the transition to symptomatic UTI.…”
