Acinetobacter baumannii has emerged as a problematic nosocomial pathogen due to its antibiotic resistance as well as its ability to colonize and cause serious infection among patients. This study aimed to evaluate the ability of A. baumannii to form biofilms as well as to investigate the antibacterial activity of cinnamaldehyde against carbapenemresistant strains of A. baumannii. A total of 23 A. baumannii clinical strains were screened for their ability to form a biofilm using tissue culture plate method. Cinnamaldehyde antibacterial ability was investigated on planktonic cells and its biofilm inhibition ability was tested. Scanning electron microscopy (SEM) was applied to confirm the antibiofilm effect of cinnamaldehyde. Biofilm formers (86.95%) were categorized into strong (17.39%), moderate (52.17%), and weak (17.39%). Cinnamaldehyde showed a strong antimicrobial activity against planktonic cells of A. baumannii at low concentrations. The best antibiofilm activity was achieved at ½ minimum inhibitory concentration (MIC) and ¼ MIC causing inhibition percentages ranging from 49.5% to 71.2% and 18.5% to 29.6%, respectively. Cinnamaldehyde exerted strong antimicrobial and antibiofilm properties indicating their potential therapeutic value that can be used as an option for treating biofilm associated clinical problems caused by A. baumannii.

Purpose The rise of carbapenem-resistant A. baumannii (CRAB) is considered a public health problem limiting the treatment options. Our current work studied the emergence and mechanisms of colistin-resistance among CRAB isolates in Egypt. Materials and Methods Seventeen clinically recovered A. baumannii were identified and screened for their antimicrobial susceptibilities using VITEK-2 system. Colistin susceptibility was evaluated using broth microdilution, and characterization of carbapenem/colistin resistance determinants was performed using whole-genome sequencing (Illumina MiSeq). Results About 52.9% (9/17) were colistin-resistant. PCR results revealed that all isolates carried bla OXA-51-like genes , bla OXA-23-like was detected in 82.3% (14/17) and bla NDM in 23.5% (4/17). Two isolates harboured bla GES-35 and bla OXA-23 . Furthermore, genome analysis of seven isolates revealed six belonged to international clone 2 (IC2) while the remaining isolate was a singleton (ST158), representing a clone circulating in Mediterranean/Middle Eastern countries. Conclusion The emergence and high incidence of colistin-resistance among CRAB clinical isolates in Egypt are alarming because it further limits therapy options and requires prudent antimicrobial stewardship and stringent infection control measures. Whole-genome sequence analyses suggest that the resistance to colistin was associated with multiple mutations in the pmrCAB genes. The high incidence of the high-risk lineage IC2 harbouring bla OXA-23-like as well as bla NDM is also of concern.

Background: Carbapenem resistant Acinetobacter baumannii has emerged worldwide in the hospitals especially in Intensive care units (ICU). Objectives: to detect carbapenems resistance and characterization of some carbapenem resistant genes among acinetobacter baumannii isolated from patients in ICU unit in Giza Chest Hospital, Egypt. Methodology: This study was designed to detect the prevalence of carbapenem resistance among A. baumannii species isolated from patients having nosocomial infections in Egypt. Antimicrobial susceptibility pattern was done. Identification and susceptibility testing was confirmed by VITEK® 2 Compact system. Isolates were further tested by the modified Hodge test (MHT) to detect carbapenem resistance. PCR was used to detect some carbapenem resistant genes. Results: A. baumanniii represent 22(16.1%) of total isolates causing nosocomial infection in ICU. A. baumanniii were resistant to sulfamethoxazole-trimethoprim, amoxicillin/ clavulanicacid, ciprofloxacin, piperacillin/tazobactamandceftazidime 100%, 90.9%, 90.9, 90.9%, 81.8% and 81.8% respectively. Imipenem resistance was 14/22(63.6%). Colistin showed the highest activity against A. baumannii isolates; the resistance rate was 4.5%. Total number of carbapenem-resistant gram-negative isolates was (38.9%). Regarding A. baumanniii 14 out of 22(63.6%) were carbapenem resistant as detected by the antibiotic susceptibility test. According to this antibiogram result, 14 A. baumanniii isolates screened for carbapenemase production by MHT, carbapenemase activity was detected among 10(71.4%) of carbapenem-resistant A. baumanniii isolates. Molecular detection of carbapenem resistant genes showed that Bla OXA-23 was the common detected gene 6/14 (42.8%). Bla OXA-58 was detected among 1/14(7.1%) of isolates. Conclusion: A. baumannii with multidrug resistance become a problem especially in immunocompromised patient. Resistance among A. baumannii caused by several mechanisms. OXA-23-like carbapenemase-producing strains have been among the most detected patterns.
