High blood pressure is a major risk factor contributing to death and disability rates in the Baltic states. The aim of this study was to compare the utilization of antihypertensive medicines in Estonia, Latvia and Lithuania from 2008 to 2018. In this retrospective cross-national comparison, nationally representative wholesale data from the IQVIA National Retail Audit were analyzed. The utilization of inhibitors of the renin–angiotensin system, beta blockers, calcium channel blockers, diuretics and centrally acting antihypertensives by Defined Daily Doses per 1000 inhabitants and day (DDD/TID) was used to calculate utilization. Time series analysis was used to analyze trends. The utilization increased annually by 10.88, 8.04 and 6.42 DDD/TID in Estonia, Latvia and Lithuania, respectively, from 2008. The utilization of antihypertensive drugs in 2018 was 372, 267 and 379.5 DDD/TID, respectively. Inhibitors of the renin–angiotensin system were the most commonly used class in 2008 and 2018. From 2008, the utilization of beta blockers and fixed-dose combinations including renin–angiotensin system inhibitors increased substantially, while that of calcium channel blockers decreased. Country-specific utilization trends were noted; e.g., the utilization of centrally acting antihypertensives was 30.9 DDD/TID in Lithuania compared to 3.01 DDD/TID in Estonia and 16.17 DDD/TID in Latvia. The use of antihypertensive medicines increased over the study period, but the trends for the different drug classes differed between countries.

Hipertenzija predstavlja jedan od najznačajnijih faktora rizika za nastanak kardiovaskularnih bolesti (KVB) na koji se može uticati. Kod određenog broja bolesnika regulisanje hipertenzije se može postići kombinovanjem antihipertenziva-primenom dva leka istovremeno ili primenom fiksne kombinacije dve lekovite supstance u jednom doziranom obliku. Cilj: Cilj rada je bio da se analizira potrošnja fiksnih kombinacija lekovitih supstanci iz podgrupe anatomsko-terapijsko-hemijskog (engl. anatomical-therapeutic-chemical-ATC) sistema klasifikacije lekova C09 (lekovi koji deluju na renin-angiotenzin sistem-RAS), u Republici Srbiji, u periodu od 2010. do 2015. godine. Takođe, cilj je bio i da se dobijeni rezultati uporede sa potrošnjom istih lekova u Kraljevini Norveškoj, Republici Finskoj i Kraljevini Danskoj, za isti vremenski period. Materijal i metode: Pri izradi rada korišćen je koncept ATC klasifikacije i definisane dnevne doze (DDD). Podaci o potrošnji lekova preuzeti su iz godišnjih izveštaja Agencije za lekove i medicinska sredstva Srbije (ALIMS), Norveškog instituta za javno zdravlje-Folkehelseinstituttet, Finske Agencije za lekove-Fimea i Danske Agencije za zaštitu podataka-Sundhedsdatastyrelsen. Količina potrošenih lekova izražavana je kao broj DDD na 1.000 stanovnika na dan (DDD/1.000 stanovnika/dan). Rezultati: Potrošnja fiksnih kombinacija inhibitora angiotenzin konvertujućeg enzima (engl. angiotensin-converting enzyme-ACE) i diuretika (C09BA) u 2010. godini, u Srbiji, iznosila je oko 14%, dok je u poslednjoj posmatranoj godini ABSTRACT Introduction: Hypertension is one of the most significant risk factors for the development of cardiovascular disease (CVD) that can be mitigated. Consumption of single products versus fixed-dose combination medicines for hypertension and hyperlipidemia during 2015–2019 in South Korea

Background Fixed-dose combinations can simplify prescribing, and numerous combination products exist for hypertension and dyslipidemia in South Korea. This study’s aim was to compare trends in the consumption of single products versus fixed-dose combinations for hypertension and hyperlipidemia. Methods and findings We analyzed the Korean national health insurance claims database from January 2015 through December 2019. Consumption of medicines was calculated using the defined daily dose per 1,000 inhabitants per day (DIDs) and expenditures over time. During 2015–2019, the use of antihypertensive drugs increased with an annual growth rate (AGR) of 0.9% for single products and with an AGR of 35.6% for fixed-dose combinations. A notable increase was observed for antihyperlipidemic combination drugs with an AGR of 268.1% compared to single products with 35.7%. For older adults (65+ years), the consumption of drugs for hypertension and hyperlipidemia was 3–4.5 and about 3 times higher, respectively, than in adults aged 20–64 years, and a sharp increase was found in antihyperlipidemic fixed-dose combinations among older adults. A large increase was seen for C09 (agents acting on the renin-angiotensin system) with an AGR of 36.5%, especially C09DB (angiotensin II receptor blockers + calcium channel blockers) was widely used and steeply increased with 114.2%. For antihyperlipidemic drugs, C10AA (HMG CoA reductase inhibitors) accounted for a large share and sharply increased, with 52.1 DIDs in 2019 and with an AGR of 78.4%, whereas C10BA (combinations of various lipid modifying agents) increased 9.6 times from 2.9 DIDs (96 million USD) in 2015 to 27.7 DIDs (912 million USD) in 2019. Conclusion The findings of increased consumption and drug spending among older adults underscores the need for real-world evidence about health outcomes of fixed-dose combinations in this population.
