Previous studies found that the risk of a femoral fracture in residents newly admitted to nursing homes was highest during the first months after admission and declined thereafter. Many nursing home admissions are preceded by a hospitalization. Therefore, the present study aimed to analyze if a similar risk pattern of fall-related fractures could also be observed in community-dwelling people at home after discharge from the hospital. Routine data of more than 690,000 German people aged 65 years and older with more than 2 million hospital discharges were used to calculate fracture rates in the first 6 months after hospitalization, for people discharged to live in the community. Incidence rates of femoral fractures as a function of time since discharge from hospital were analyzed. Analyses were stratified by sex, age, the need for care, and diagnostic groups. For femoral fractures the incidence was highest during the first months after discharge and declined thereafter. This pattern was observed in women and men, in different age-groups, in different diagnostic groups, and in people with and without the need for care. For example, rates for femoral fractures in women declined from 17.4 to 11.0 per 1000 person years over the first 6 months after admission, and in men over the same time period from 8.2 to 4.5 per 1000 personyears, respectively. We conclude that the first weeks at home after discharge from the hospital are associated with an increased risk for femoral fractures. ß

The bisphosphonates are drugs for synthetics used without treatment of neoplasms and bone, Paget's disease and more reason for excitement in cases of postmenopausal osteoporosis. These drugs resemble some known side effects, however, was recently identified a new complication, with oral manifestation, called Osteonecrosis. When this necrosis of the bone bases is associated to the use of Bisphosphonates, gets its name from Osteonecrosis of MS would expire by Bisphosphonates. The type of bisphosphonate, a route of administration and duration of treatment with these drugs seem to have direct relation with the incidence of osteonecrosis associated with bisphosphonates. The aim of this work was to present a revision last five in relation to available bisphosphonates groups Brazil, emphasizing those used in the treatment of Osteoporosis, associating them to Osteonecrosis of would expire and as possible prior to diagnostic aspects from image Diagnostics. Peak bone mass for spine and hip is reached in the mid-twenties and adolescents should be counselled on adequate nutrition to ensure sufficient calcium intake, regular weight-bearing exercise, maintaining normal body weight, avoiding smoking and limiting alcohol intake. These measures are important to prevent osteopenia and osteoporosis by obtaining a maximum peak bone mineral density (BMD) and to maintain it by avoiding excessive bone loss. One year before the onset of menopause, however, as a result of oestrogen deficiency, there is an increase in osteoclastic activity without a similar increase in osteoblastic activity, resulting in accelerated bone loss. The average decrease in BMD during the menopausal transition is estimated to be about 10% and a woman's risk of sustaining an osteoporotic or fragility fracture doubles for each decade after the age of fifty. Half of women older than 50 years of age will be osteopenic compared to 10% who are osteoporotic and only 40% will have normal BMD. This article gives an overview of the prevention, diagnosis and management of osteoporosis during and before menopause.
