Antidepressants and Incident Hypertension in Primary Care Patients

Abstract: Objective: Many ADMs can alter blood pressure (BP), but the research on the effect of antidepressant medication (ADMs) on incident hypertension is mixed. We investigated whether the use of ADMs was associated with the subsequent development of hypertension. Methods: A retrospective cohort study was conducted using electronic medical record data from 6224 patients with primary care visits from 2008 to 2015. Prescription orders were used to identify ADM use, and hypertension was defined by medical record diagnosi… Show more

“…Tricyclic antidepressants (TCA) and serotonin-norepinephrine reuptake inhibitors (SNRI) are traditionally considered to have hypertensive effects and were shown to be associated with an increased risk of incident hypertension [ 66 •, 67 ••]. The BP effects of SNRI seem to be dose-dependent and become more relevant at supratherapeutic doses [ 68 , 69 ].…”

“…In contrast, some conflicting results were published. For example, Breeden et al [28] found that antidepressant use was not associated with incident hypertension after controlling for associated cardiovascular factors. Katsi [4] reported that the effects of venlafaxine on blood pressure were similar to placebo and that only rare reports of hypertension were found with milnacipran.…”

“…Contudo, um estudo publicado pelo Journal of the american board of family medicine identificou que as alterações de pressão arterial pelo uso de IRSN estão associadas mais ao uso de doses consideradas supra terapêuticas, ou seja, essas alterações na PA não são tão significativas estatisticamente devido que, as doses utilizadas normalmente nos pacientes são mais baixas (BREEDEN et al, 2018).…”
