Anticoagulation Therapy in Specific Cardiomyopathies: Isolated Left Ventricular Noncompaction and Peripartum Cardiomyopathy

Abstract: In 2 distinct entities, left ventricular noncompaction (LVNC) and peripartum cardiomyopathy (PPCM), routine anticoagulation therapy is often used in current practices. However, our systematic review showed that LVNC itself was not associated with the increase in thromboembolism event rates and therapeutic anticoagulation therapy should not be considered only for LVNC, unless there is risk factor for thromboembolism. Current literature justifies prophylactic therapeutic anticoagulation in LVNC with low left ven… Show more

“…Interestingly, in our case, the child developed recurrent thrombosis despite anticoagulation therapy, while the mother received anticoagulation therapy only for a limited period, due to very decreased EF, without any thrombotic events in her evolution whatsoever. There are no specific guidelines regarding anticoagulation therapy in ILVNC; however, it is justified in the setting of associated systolic dysfunction/atrial fibrillation or thromboembolism history, including intracardiac thrombi ( 23 26 ).…”

“…The incident rate of thromboembolism in LVNC is controversial (1,2,18). Anticoagulation with warfarin is recommended in LVNC patients with history of thromboembolism, atrial fibrillation, and/or a reduced LV function (LV ejection fraction <40%) (19). In addition, anticoagulation is indicated for thrombus in the LV as first-line therapy according to the JCS guideline (20).…”

“…we did not sub-analyze intracardiac thrombi according to etiology (i.e., stress-induced cardiomyopathy, isolated left chamber noncompaction, peripartum cardiomyopathy, inflammatory bowel disease, Behcet Disease, leukemia) [9][10][11]. Moreover, as there was no significant correlation between thrombus location and ST, thrombi in the left atrium were not distinguished from those in the left ventricle.…”
