Antibodies provoked by the transfusion of biotin‐labeled red cells

Abstract: Biotinylation does not render RBCs reactive with normal human plasma (i.e., presumably does not evoke neoantigens). Transfused B-RBCs occasionally provoke IgG antibodies in healthy subjects. Because the biologic effects of B-RBC antibodies currently are unknown, testing for them is recommended when multiple B-RBC transfusions are given to study RBC volume or circulating kinetics.

“…Indeed, in one of our studies, antibodies to BioRBC developed in 3 of 20 adults transfused with autologous BioRBC [87]. To date a total of 4 adult study subjects have developed antibodies to BioRBC 3 to 4 months post-transfusion [57, 88]. None had evidence of hemolysis, change in the BioRBC survival, abnormalities of their peripheral blood count or RBC or reticulocyte indices, or untoward clinical signs or symptoms.…”

“…In addition, the best method for detecting antibodies to BioRBC has yet to be determined. Our groups initially used an assay for BioRBC antibodies based on agglutination in test tubes [88]. However, we have recently developed a more sensitive anti-IgG antibody method using gel cards (manuscript in preparation).…”

“…The major potential disadvantage of the biotin label is the development of antibodies against biotinylated RBC. In one report [23] transient antibodies were detected using an agglutination technique in 3 of 20 subjects that were given biotin-labeled RBC. None of these antibodies appeared to have an effect on RBC lifespan, and none could be detected after 1 year.…”

“…Biotinylation of erythrocytes is also readily achieved, and biotinylated erythrocytes can be safely reinfused into humans (40)(41)(42). This procedure would allow specific conjugation and multimerization of biotinylated viral receptors on the erythrocyte surface by using avidin.…”
