Antibiotic Susceptibility of Enterohemorrhagic Escherichia coli O157:H7 Isolated from an Outbreak in Japan in 1996

Abstract: The antibiotic susceptibilities of 43 strains of Escherichia coli O157:H7 identified in the summer of 1996 in Japan were investigated. Growth of 90% of O157 strains was inhibited at a concentration of ≤0.5 μg/ml by several agents including fosfomycin with glucose-6-phosphate.

“…The mainstay of the therapy for hemorrhagic colitis is the management of dehydration, electrolyte abnormalities and gastrointestinal blood loss. The antibiotic susceptibilities of EHEC have been investigated [33]. However, no controlled trials of antibiotic treatment of EHEC infection have been reported.…”

“…An in vitro investigation of antibiotic susceptibilities during the 1996 outbreak in Osaka, Sizuoka and Tokyo showed that the growth of 90% of 43 strains of E. coli O157 collected from 97160 stool specimens was inhibited at a concentration of ≤0.5 μ g/mL by 15 of 24 antimicrobial agents tested, which included new quinolones, cephalosporins (except cefaclor) and fosfomycin with 3 μ g/mL of G6P. G6P is released from serum into the intestine of patients with O157 and induces the transport of fosfomycin into the bacteria 47 …”

“…In 1996, several large outbreaks of EHEC O157:H7 infection occurred in Japan affecting more than 9000 people and causing 11 deaths 47 . Although treatment with antibiotics is not considered proven to be efficacious for this infection, the Japanese Ministry of Health and Welfare recommended that fosfomycin, norfloxacin or kanamycin be administered orally to children and that fosfomycin or a new quinolone be given to adults orally during the initial stage of infection 47 . A survey study targeting physicians who treated these patients revealed that most of the patients (95.9%) received antimicrobial agents and that fosfomycin comprised 84% of the prescribed treatment 32 …”
