Antibiotic Penetrance of Ascitic Fluid in Dogs

Abstract: Antibiotic concentrations in ascitic fluid after parenteral therapy may be important in the treatment of peritonitis. We have created ascites in dogs by partial ligation of the inferior vena cava. Ascitic fluid volume was measured at the time each antibiotic was administered. Nine antibiotics were studied in the same three dogs. Antibiotic concentration in ascitic fluid was found to vary inversely with ascites volume. Percentage of penetration (ratio of ascites peak to serum peak ×100) ranged from 5.8 to 65% a… Show more

“…On the 10 occasions when paracentesis was performed during SBP, peritoneal fluid genta-micin levels ranged from <2.0 to 5.7 ,ug/ml (mean of 4.2 ,ug/ml for the nine patients whose levels were detectable); simultaneous serum concentrations ranged from 4.0 to 8.2 uLg/ml (mean, 6.1 ,Lg/ml) ( Table 2). The percent penetration of gentamicin into peritoneal fluid averaged 67.8%, with a range of 35.9 to 101.2%, for the nine instances when this could be calculated.…”

“…Because gram-negative bacilli are etiological in the majority of cases of SBP (1, 2, 11), aminoglycosides are commonly used in the treatment of SBP. However, the adequacy of transport of aminoglycosides into cirrhotic ascites has received only limited study (4), and experimental studies in animals have yielded conflicting data (5,6,8). The purpose of the present study was to determine simultaneous serum and peritoneal fluid gentamicin concentrations 1 and 4 h after administration of this drug in patients with SBP.…”

“…We have previously measured ascitic fluid concentrations of three cephalosporins in a dog model after a single parenteral dose (6). In this paper we extend these observations to include cefamandole in addition to cefazolin, cephaloridine, and cephalothin in a multiple-dose study of serum and ascitic fluid pharmacokinetics and protein binding of cephalosporins in dogs.…”

“…Ascites was produced in mongrel dogs (initial weight, 14 to 18 kg) by partial ligation of the inferior vena cava as previously described (6). Dietary sodium chloride supplementation (15 g/day) was required in one dog to maintain ascites.…”

“…These tics of pleural fluid in our patients suggest the presence of acute severe inflammation in pleural space, even though cell counts were not carried out. Third, Gerding et al (4) have reported that the levels of antibiotic in ascitic fluid of dogs are related inversely to the volume of ascitic fluid. The large volumes of extravascular fluid, i.e., 4 to 6 liters, could have disturbed circulation and drug kinetics, which might inhibit drug penetration into extravascular fluid.…”
