Antibacterial Properties of Gemifloxacin and Trovafloxacin in Urine Ex Vivo: Phase I Study

Abstract: Urine bactericidal titers (UBTs) against Escherichia coli ATCC 25922 and Staphylococcus saprophyticus ATCC 1970 were determined after the administration of single oral doses of gemifloxacin at 320 mg and trovafloxacin at 200 mg to healthy volunteers. Gemifloxacin presented significantly lower experimental versus mathematically predicted UBTs over 72 h, due to the effect of urine on the susceptibility of the E. coli strain. Experimental UBTs were significantly higher for gemifloxacin than trovafloxacin against … Show more

“…For urine, a 5‐ to 10‐fold higher C max / MIC ratio was measured, because of the 5‐ to 10‐fold higher urine concentrations of tobramycin. Validation of these findings in clinical trials is also needed, as an increase in MIC values in urine can occur, depending on several factors, including urine pH, which could be less alkaline after intense exercises or diseases (Garcia‐Calvo et al. , 2001; Lees, 2004).…”

“…The penetration of gemifloxacin into human polymorphonuclear leukocytes was examined by exposing polymorphonuclear leukocytes incubated in Hanks' balanced salt solution to various concentrations (0.1-25 µg/ml) of radiolabeled gemifloxacin. 58 The intracellular concentrations of gemifloxacin were up to 8 times higher than that of the extracellular fluid. The uptake of gemifloxacin was rapid, reversible, and nonsaturable.…”

“…There is a modest relationship between total T > MIC in plasma and treatment outcome in urinary tract infections [48]; and antibacterial activity in urine, rather than urinary concentrations and determination of the MIC, may be better correlated with outcomes in uncomplicated cystitis. In this sense, postmarketing phase I studies measuring ex-vivo urine bactericidal activity and urine killing rates against selected strains with specific resistance traits may help to establish adequate dosing intervals, length of treatment or define susceptibility/resistance of prevalent or not prevalent specific phenotypes [49][50][51][52].…”
