Anti‐TNF‐α therapy improves insulin sensitivity in non‐diabetic patients with psoriasis: a 6‐month prospective study

Abstract: Our results support a beneficial effect of the anti-TNF-α blockade on the mechanisms associated with accelerated atherogenesis in patients with psoriasis.

“…Table summarizes the epidemiological and clinical features of these 29 patients immediately prior to the onset of adalimumab therapy. As previously reported, at 6 months after the onset of adalimumab a significant reduction ( P < 0.05 for each comparison) in all of the markers of disease activity and severity including ESR, usCRP, BSA, PASI, NAPSI, PGA and PASE was observed . In this regard, BSA and PASI experienced a significant reduction (BAS from 37.9 ± 16.3% to 3.1 ± 5.5; PASI from 18.9 ± 7.8 to 1.3 ± 2.1).…”

“…Arthralgia was often described by the patients but none of them had a clinical pattern of psoriatic arthritis. The recruitment protocol has been previously described . Briefly, psoriatic patients with diabetes, kidney disease, hypertension or body mass index of 35 kg/m 2 or more were excluded.…”

“…The hypothesis that anti‐TNF‐α therapy can have a beneficial effect in the risk of atherosclerosis is getting stronger. In a cohort of 29 patients treated for 6 months with adalimumab therapy, a statistically significant improvement ( P = 0.008) of insulin sensitivity was observed after adalimumab treatment . The same authors showed that anti‐TNF‐α therapy achieved statistically significant reduction ( P = 0.0001) of retinol‐binding protein 4 (RBP4) serum levels.…”

“…We consecutively enrolled patients with moderate‐to‐severe psoriasis over an 18‐month period from the Dermatology outpatient clinics of the University Hospital Marques de Valdecilla (Santander, Northern Spain). Recruitment protocol has been previously described . Psoriatic patients with diabetes, kidney disease, hypertension or body mass index ≥ 35 kg/m 2 were excluded.…”

“…Previously, in a prospective study of psoriatic patients with active disease, we disclosed a beneficial effect of adalimumab, a fully human monoclonal antibody targeted against TNF‐α, on the mechanism associated with accelerated atherogenesis, specifically on insulin resistance …”
