Anti-psychotics adherence among out-patients with schizophrenia in Hong Kong

Abstract: Non-adherence is a widespread phenomenon affecting a significant proportion of outpatients under treatment. Clinicians have relatively low sensitivity in detecting which patients are non-adherent. Non-adherence behavior is predicted by level of awareness of illness, perceived benefits of medication, negative feelings toward medication and age.

“…Among various factors such as long duration and serious adverse effects of medication and lack of social and family support [5,6], a lack of insight into the illness and medication taking is one of the most important inhibiting factors influencing medication adherence to antipsychotics and other treatments in patients with schizophrenia spectrum disorders [7]. In addition, acute and severe psychopathology (for example, suspicions and delusional beliefs), and any delay in seeking treatment during the early stages of schizophrenia, can render it difficult for prompt and efficacious interventions and understanding about the significance of medication adherence, thus reducing the likelihood of their adherence behaviors [6,8].…”

“…[28] This may be attributed to the distrust of young patients, in the first stages of disease, of the diagnosis and their need for treatment. However, other studies stated that older patients are at increased risk of non-adherence due to age related factors such as workingmemory loss and impaired executive performance, in addition to their administering multiple medications.…”

“…13 regarding patient's age, younger patients show higher non-adherence rates than older ones. 50 Faith healers (9.61%), lack of family education (7.69%), were the most important psychosocial reasons for non-adherence to psychiatric medicines. 51 Compliance from the family members is also a dependent factor in non-adherence as has been revealed.…”
