Cistanoside A (Cis A), an active phenylethanoid glycoside isolated from Cistanche deserticola Y. C. Ma, has received our attention because of its possible role in the treatment of osteoporosis. In the present study, we evaluated the effects of Cis A on an ovariectomized (OVX) mice model and investigated its underlying molecular mechanisms of action. After 12 weeks of orally-administrated intervention, Cis A (20, 40 and 80 mg/kg body weight/day) exhibited significant antiosteoporotic effects on OVX mice, evidenced by enhanced bone strength, bone mineral density and improved trabecular bone microarchitecture. Meanwhile, the activities of bone resorption markers, including tartrate-resistant acid phosphatase (TRAP), deoxypyridinoline (DPD) and cathepsin K, were decreased, and the bioactivity of bone formation marker alkaline phosphatase (ALP) was increased. Mechanistically, Cis A inhibited the expression of TNF-receptor associated factor 6 (TRAF6), an upstream molecule that is shared by both nuclear factor kappa-light chain enhancer of activated B cells (NF-κB) and phosphatidylinositol 3-kinase (PI3K)/Akt pathways and subsequently suppressed the levels of receptor activators of nuclear factor kappaB ligand (RANKL), downregulated the expression of NF-κB and upregulated osteoprotegerin (OPG), PI3K and Akt, which means Cis A possessed antiosteoporotic activity in ovariectomized mice via TRAF6-mediated NF-kappaB inactivation and PI3K/Akt activation. Put together, we present novel findings that Cis A, by downregulating TRAF6, coordinates the inhibition of NF-κB and stimulation of PI3K/Akt pathways to promote bone formation and prevent bone resorption. These data demonstrated the potential of Cis A as a promising agent for the treatment of osteoporosis disease.

Background: Leptin has a great effect on bone through direct or indirect involvement in bone remodeling. Considering the ambiguities that exist regarding the effect of leptin on bone and bone-related diseases including osteoporosis, in this study, we aimed to conduct a systematic review of various studies on the effect of leptin on osteoporosis, which may find an answer to the existing ambiguities. Methods: The search was performed to review studies on the effects of leptin on osteoporosis by using several databases including Scopus, PubMed, Web of Science, and Google Scholar. Electronic searches were conducted on 5 Jan 2023. There was no limit on the publication date of the articles. The risk of bias for the animal study was assessed with the CAMARADES checklist, and the study quality assessment was also assessed based on the guidelines for in vivo experiments (ARRIVE). In this study, the risk of bias (quality) of human studies was assessed using the quality assessment checklists by NHLBI. Results: Overall, 34 articles were included for data extraction and quality assessment. Overall, 27 human studies and seven animal studies were included in the article. The results of most of the studies conducted in this study showed that leptin has a physiological role in maintaining bone mass and better bone quality and reduces bone marrow adipogenesis and increases bone mineral density (BMD). As plasma leptin levels increased, BMD values or bone formation biomarkers increased. Conclusion: Leptin has an inhibitory role against bone resorption and increasing osteoprotegerin (OPG) levels, which, as a result, maintains bone density and reduces osteoclast activity, and has a positive relationship with increasing osteocalcin.

Purpose: This study aimed to determine the therapeutic effect of Bushen Jiangu Decoction (BJD) on osteoporosis (OS) in rats. Methods: The animals were divided into the following groups: control group, rats received no treatment; ovariectomy (OVX) group, rats subjected to OVX surgery and were treated with normal saline; OVX + Fosamax group, rats that received OVX and were treated with 2 mg/kg/week of Fosamax; low-dose BJD (40 mg/kg/day), medium-dose BJD (80 mg/kg/day) and high-dose BJD (160 mg/kg/day) groups, where rats received OVX surgery with BJD doses of 40, 80 and 160 mg/kg/day, respectively. At four weeks after treatment with OVX, Fosamax or BJD was orally administered for four months. Bone mineral density (BMD) was assessed, while serum hormone, serum levels of follicle-stimulating hormone (FSH), alkaline phosphatase (ALP), estradiol (E2), luteinizing hormone (LH), osteocalcin (OC) and telopeptides of collagen type I (CTx) were measured by enzyme linked immunosorbent assay (ELISA). Results: The results demonstrated that the reduced BMDs of L4 femurs and vertebrae were inhibited by BJD. Furthermore, BJD significantly elevated the serum levels of FSH, E2 and LH in OS rats. Furthermore, the serum CTx, ALP and OC levels of these rats dramatically decreased, when compared to the OVX group. Conclusion: These findings suggest that BJD can improve OVX-induced OS.
