Anti-nucleosome antibodies outperform traditional biomarkers as longitudinal indicators of disease activity in systemic lupus erythematosus

Abstract: Anti-nucleosome antibodies demonstrate greater fidelity as a biomarker for changes in SLE disease activity than traditional biomarkers, supporting the routine monitoring of this antibody in clinical practice.

“…The findings reported in this study may therefore have important clinical implications for improved diagnosis and management of SLE. In many studies, increased disease activity has been shown to generally correlate with higher levels of ANAs and anti‐dsDNA antibodies . However, in most patients, while there is the presence of serologically active autoantibodies, disease activity remains quiescent .…”

“…By two-tailed t test, the median cfDNA levels in patients with inflammation were not significantly higher compared with patients without inflammation many studies, increased disease activity has been shown to generally correlate with higher levels of ANAs and anti-dsDNA antibodies. 28,29 However, in most patients, while there is the presence of serologically active autoantibodies, disease activity remains quiescent. 30 Thus, in addition to routine monitoring the titres of patient-specific autoantibodies, the simultaneous assay of cfDNA could also be used as a simple screen for rapidly evaluating changes in disease activity.…”

“…Priority of anti-nucleosome immune response compared to anti-DNA and anti-histone ones is indirectly conirmed by revelation of earlier subtype of anti-nucleosome antibodies that do not interact with both DNA and histones [35]. There is close association of these antibodies with SLE activity and the kidney involvement [36].…”

“…Further, they can be found in 10% of patients with lupus nephritis who are anti-dsDNA antibody negative. They are useful in the follow-up of lupus patients, and they have shown to be even better than anti-dsDNA antibodies [28]. However, they are not widely available; thus, their use in clinical practice is limited.…”
