SUMMARYAnti-neutrophil cytoplasmic antibodies (ANCA) in sera from ulcerative colitis (UC) patients have been described as reacting with proteins in the granules of human neutrophils such as cathepsin G and lactoferrin and with yet unidentified antigens. Here we report the existence of a new member of perinuclear ANCA (P-ANCA) in UC patients. In the previous study, we found that UC patients had a novel P-ANCA against neutrophil 28-kD protein. In this study, we purified the same antigens from HL-60 lysates by using reversed phase high-performance liquid chromatography, and revealed that the 28-kD antigen consisted of two different proteins. The N-terminus amino acids of these proteins are identical with those of high mobility group (HMG) non-histone chromosomal proteins HMG1 and HMG2. Immunoblotting analysis of human neutrophil lysates using rabbit anti-HMG1/2 antisera revealed a single band of 28 kD, and the 28-kD band detected by immunoblotting analysis using patient's serum IgG completely disappeared after preincubation with a mixture of HMG1 and HMG2. Furthermore, rabbit anti-HMG1/2 antisera showed a perinuclear staining pattern in indirect immunofluorescence studies using ethanol-fixed neutrophils. These data demonstrate that HMG1 and HMG2 are novel target antigens of P-ANCA. HMG1 and HMG2 are distributed in the nuclei and cytoplasm of eukaryotic cells and act as transcription factors. Their intracellular localization and functions are distinct from those of the previously reported granular antigens of P-ANCA.

SUMMARYWe analysed the clinical significance of ANCA in patients with ulcerative colitis. On either an indirect immunofluorescence assay or an ELISA with fixed neutrophils, 71% (25/35) of the patients were positive for ANCA. However, only half of them reacted with either cathepsin G or lactoferrin. Western blot assays revealed positive bands in 40% (10/25) of the antibody-positive patients. The sizes of the bands detected were 58, 47, 44, 40, and 28 kD. No significant correlation was found between the ANCA positivity and various variables, i.e. disease activity, extent of lesion, or treatment of the disease. The anti-cathepsin G and 28-kD positivity, however, significantly correlated with a refractory type of ulcerative colitis.

INTRODUCCIÓNLa pancreatitis autoinmune (PA), es un término descrito por primera vez en 1995 por Yoshida y cols. (1) También ha sido denominada en la literatura médica como pancreatitis linfoplasmocitaria esclerosante, pseudotumor pancreático, pancreatitis primaria inflamatoria o pancreatitis crónica no alcohólica con destrucción ductal (2).Es una entidad muy infrecuente, con aproximadamente 100 casos descritos en Literatura. La PA constituye cerca del 5% de los casos de pancreatitis crónica (3). Así mismo el 5% de los casos de cirugía pancreática corresponde a patología benigna, entre los que se encuentra esta entidad (4).El interés por esta patología en los últimos años se ha incrementado por las mayores posibilidades diagnósticas debido al uso de nuevos marcadores de la enfermedad y nuevas técnicas de biopsia pancreática (5,6).Presentamos un nuevo caso de P.A, para mostrar la importancia de la sospecha diagnóstica precoz debido a la excelente respuesta al tratamiento adecuado. CASO APORTADOMujer de 33 años de edad, sin antecedentes previos de interés. Presentó cuadro de 3 semanas de evolución de prurito intenso, astenia, anorexia y pérdida de peso, al cual se asocia ictericia, coluria y acolia en los últimos días. En la exploración física destaca ictericia de piel y mucosas, siendo el resto de las exploración física normal.En la analítica presentaba Hb 11,2 mg/d, plaquetas 286.000 x 103/µl, leucocitos 9.460 x 103/µl (fórmula normal). Actividad de protrombina 105%. Glucosa 89 mg/dl; Urea 45 mg/dl; Creatinina 1,2 mg/dl; Bilirrubina total total 12,4 mg/dl; Bilirrubina directa 9,7 mg/dl; GOT 53 U/l; GPT 60 U/l; GGT 66 U/l; FA 388 U/l; Amilasa 186 U/l; LDH 292 U/l; VSG 54 mm; PCR 35 mg/l. Serología virus [0212-7199 (2008) RESUMENLa pancreatitis autoinmune es una reciente entidad a tener en cuenta en el diagnóstico diferencial de pancreatits crónica-masa pancreática. La presencia de estenosis difusa e irregular del Wirsung, junto con niveles elevados de IgG4, presencia de determinados autoanticuerpos e infiltración linfoplasmocitaria de páncreas son la clave del diagnóstico.La sospecha diagnóstica precoz y la colaboración multidisciplinar es vital en estos casos, ya que puede evitar cirugía innecesaria, puesto que el diagnóstico diferencial se establece con entidades entre las que se encuentra cáncer de páncreas, pancreatitis crónica, cirrosis biliar primaria o colangitis esclerosante primaria.Además, la pancreatitis autoinmune tiene excelente respuesta a tratamiento esteroideo, con resolución completa de parámetros clínicos, analíticos y radiológicos, como ocurría en nuestro paciente. Debido al escaso número de casos descritos en la Literatura, sería necesario la elaboración de estudios con seguimiento largo plazo para conocer pronóstico y frecuencia de asociación a otras patologías.
