Background: Diabetic nephropathy is the most common cause of end-stage renal disease. Emerging evidences indicate that many mechanistic pathways including apoptosis play an important role in the pathogenesis and progression of macrovascular and microvascular complications of diabetes mellitus. The aim of the present study is to show the effects of grape seed extract (GSE) on oxidative stress and apoptosis in the kidney of streptozotocin-induced diabetic rats.

Colistin is an old antibiotic used in the treatment of Gram-negative infections. It was once suspended because of its nephrotoxic effect but has since been reintroduced due to multidrug-resistant bacterial infections. The pathogenesis of colistin-associated nephropathy has not been clarified, and there is currently no effective therapeutic or prophylactic agent available. The aim of this study was to investigate the roles of caspase-associated apoptosis and caspase 1, calpain 1, inducible nitric oxide synthase (iNOS), and endothelial nitric oxide synthase (eNOS) expression in the pathogenesis of colistin-associated nephrotoxicity and the effect of grape seed proanthocyanidin extract (GSPE) in preventing it. Twenty-four rats were divided into three groups: control, colistin, and colistin plus GSPE (colistin؉GSPE). Colistin-associated nephropathy was induced by the administration of 300,000 IU/kg of body weight/day colistin intraperitoneally for 7 days. The experiment was discontinued on the seventh day. Blood was collected for measurements of blood urea nitrogen (BUN) and creatinine levels. Histopathological examination of kidney tissue and caspase 1 and 3, iNOS, eNOS, terminal deoxynucleotidyltransferase-mediated dUTP-biotin nick end labeling (TUNEL), and calpain 1 staining was also performed. Significant increases in BUN levels; creatinine levels; renal histopathological scores; and TUNEL, caspase 1 and 3, calpain 1, iNOS, and eNOS staining were observed for the colistin group compared to the control group. Significant decreases in BUN levels; creatinine levels; renal histopathological scores; and TUNEL, caspase 1 and 3, calpain 1, iNOS, and eNOS staining were observed in the colistin؉GSPE group compared to the colistin group. Our study shows, for the first time in the literature, that caspase-mediated apoptosis, iNOS, caspase 1, and calpain 1 are involved in the pathogenesis of colistin-associated nephropathy. GSPE had a renoprotective effect, as shown by the lowered levels of these mediators.

ÖzetAmaç: Çeşitli hormon ve aracılar idrar Na atılımını azaltırken nitrik oksid (NO) gibi birtakım aracılar ise Na atılımını arttırmaktadır. Üzüm çekirdeği proanthocynidin özü (ÜÇPÖ), NO düzeyini arttırarak antioksidan etki eden bir moleküldür. Çalışmamızda ÜÇPÖ'nin idrar Na atılımı üzerine etkisini değerlendir-meyi amaçladık. Gereç ve Yöntem: 14 adet rat kontrol ve ÜÇPÖ olmak üze-re ayrıldı. Kontrol grubuna gavaj ile 1 hafta süreli 1 cm3 süt verilirken ÜÇPÖ grubuna 100mg/kg ÜÇPÖ verildi. 7. gün 24 saat süreli metabolik kafeste takip edilen ratların idrarları toplanıp 24 saatin sonunda idrar Na atılımı çalı-şıldı ve deney sonlandırıldı. Bulgular: Kontrol ve ÜÇPÖ gruplarının kilo, aldığı katı ve sıvı gıda ve idrar volumleri arasında fark yoktu. 24 saatlik idrar Na atı-lımı ÜÇPÖ grubunda (1.43±0.30 gr/gün) kontrol grubuna (1.37±0.29 gr/gün) göre daha fazla idi ancak istatistiksel anlamlı değildi. Na atılımı ile katı gıda alımı (p=0.029, r=0.583) ve idrar hacimleri (p<0.001, r=0.806) arasında pozitif korelasyon mevcuttu. Tartışma: Literatürde ilk kez çalışmamızda ÜÇPÖ'nin sağlıklı ratlarda istatistiksel anlamlı olmasa da idrar Na atılımını arttırdığını, katı gıda alımı ve idrar hacimlerinin Na atılımını etkilediğini gösterdik. Na atı-lımında ve dengesinde bozukluk olan hipertansif ratlarda yapılacak daha ileri çalışmalarda ÜÇPÖ'nin idrar Na atılımı üzerine etkisinin değerlendirilmesinin faydalı olabileceğini düşünmekteyiz. Grape seed proanthocyanidin extract (GSPE) is a molecule that has an antioxidant effect by increasing NO levels. Our study was intended to evaluate the effect of GSPE on Na excretion. Material and Method: Fourteen rats were divided into control and GSPE groups. The control group was given 1 cm3 milk by gavage for one week, while the GSPE group was given 100 mg/kg GSPE. Seventh-day urines were collected from rats monitored over 24 h in a metabolic cage. Urinary Na excretion at the end of 24 h was investigated and the experiment concluded. Results: There was no difference between the control and GSPE groups in terms of weight, solid and liquid food intake and urine volumes. 24-hour urinary Na excretion was higher in the GSPE group (1.43±0.30 g/day) compared to the control group (1.37±0.29 g/day), although the difference was not statistically significant. Na excretion was positively correlated with solid food intake (p=0.029, r=0.583) and urine volume (p<0.001, r=0.806). Discussion: Our study shows, for the first time in the literature, that GSPE increases urinary Na excretion in healthy rats, though not to a statistically significant extent, and that solid food intake and urine volume affect Na excretion. We think that it will be useful for the effect of GSPE on urinary Na excretion in hypertensive rats with impaired Na excretion and balance to be evaluated in future studies.

Abstract: We demonstrated that GSPE prevents CsA nephropathy and that this effect is achieved by anti-apoptotic and anti-oxidant activity. We also achieved a significant recovery in kidney functions without affecting CsA plasma levels.
