Anterograde–retrograde method for harvest of distally based sural fasciocutaneous flap: Report of results from 154 patients

Abstract: Distally based sural fasciocutaneous flap is traditionally raised by the retrograde method. This article introduces the anterograde-retrograde method for harvest of the flap and describes our experience on altering the flap plan. A total of 159 flaps in 154 patients were elevated by the anterograde-retrograde approach that harvest of the flap began with exploring the peroneal artery perforators nearby the pivot point before the upper and bilateral edges of the flap were incised. Partial necrosis occurred in 16… Show more

“…Free tissue transfer can be an excellent option if local tissue transfer with a pedicled or propeller flap is unsuitable. Since its introduction by Yoshimura et al, the peroneal flap has gained considerable popularity in reconstructive surgery for most tissue defects . It is useful as a pedicle flap and as a free flap, because the procedure for elevating it is very simple.…”

“…Due to the limited mobility and availability of the overlying skin, even a small defect in the distal leg and foot region may require microsurgical reconstruction. There are many possible reconstructive options for this region, although all methods have specific limitations . With their considerable superiority over traditional conventional flaps in terms of decreased donor site morbidity, perforator flaps have become increasingly popular in reconstructive surgery in the last few decades.…”

“…In addition, it should be aesthetically acceptable and suitably thin to allow functional recovery of the lower limb. Regional flaps, such as the sural flap, have also been used frequently for lower extremity reconstruction . These flaps typically do not require microsurgical techniques or equipment .…”

“…Reverse fasciocutaneous sural flap has the advantages of easy and quick harvesting without sacrificing major arteries and can be completed in one‐stage operation. However, the failure and complication rates were reported to be in the range of 5–59% . After reviewing the literatures, two major problems were considered to influence the outcome of the reverse sural fasciocutaneous flap: patient selection and surgical technique.…”

“…Surgical modifications to increase the success rate have been described . Most articles confirmed that the flap was relatively reliable when the skin island was designed over the lower two‐third of posterior calf .…”
