Anterior urethral valves in children: A possible association between anterior urethral valves and Cowper’s duct cyst

Abstract: Abstract:To elucidate the pathophysiology of congenital obstruction of the anterior urethra and to investigate the association between anterior urethral valves and syringocele. Three boys with congenital obstruction of the anterior urethra diagnosed at our department between 1997 and 2004 were analyzed retrospectively. All three boys had congenital obstruction in the bulbar urethra. The presenting symptoms and age of each patient were varied. Whereas continuity between Cowper's duct and the diverticulum was no… Show more

“…Between 75 and 80% of patients with anterior urethral valve present with some degree of bladder dysfunction, even after adequate valve ablation is done (19). Kajiwara et al evaluated urodynamic characteristics in patients with AUV which included bladder instability, overactive bladder, and poor bladder compliance and capacity (20). …”

“…Some have been proposed as possible mechanisms that are related to embryonic urethral development defects and the valve formed by excessive urethral tissue during embryonic development (11). It has also been reported that AUV and diverticulum formation may result from the distal tip of a ruptured syringocele (12,13). AUV is manifested as a mucosal fold in the urethra, located generally along the ventral surface of the urethra.…”

“…Acquired diverticula are only slightly more frequent than congenital ones. In 50% of these cases peri-urethral suppuration was the promoting etiology (other etiologies are: anterior urethral valves in children [1,2] and ectopic ureter). All were found to have a posterior urethral diverticulum thought to represent the remains of the original recto-urethral fistula accompanying the high rectal atresia [4].…”

“…The anterior urethra and peno-scrotal junction were the most commonly affected sites. Complications included urethra-cutaneous fistula [5], urethral stricture [2] and wound infection [4]. …”
