Anterior tension-free repair under local anesthesia of abdominal wall hernias in continuous ambulatory peritoneal dialysis patients

Abstract: The absence of surgical and general complications and the nearly immediate resumption of PD indicate the anterior tension-free repair under local anesthesia as a safe and effective technique for CAPD patients even in an ambulatory or day-surgery setting.

“…However, more modern cohorts show a rising incidence of umbilical hernias (19). Surgical treatment was well tolerated in our study, and the 10% recurrence rate is comparable to rates in older series (6)(7)(8)(9)(10)20). Only 7% of our patients undergoing surgery had to convert permanently to hemodialysis, a proportion similar to those reported by other authors (2,10).…”

“…The anesthesiologists had handled the peripheral nerves in the members but not in the abdomen because they have had the epidural or either subarachnoid anesthesia, 7 However, sometimes we have older patients with high surgical risk to operate, so have been sought more secure anesthetic techniques. 8 Over the years the techniques of abdominal peripheral block have been refined, 9 For more than forty years, inguinal repair has been performed local anesthesia, offering less postoperative pain 10,11,12 being an approach simpler and secure, the procedures performed have been used with good results, offering the advantages in addition to producing less postoperative pain. 13 In our hospital, Dr. Garrido has performed more than 100 appendectomy with local anesthesia with the same results as we described below, so we decided to report this case.…”

“…In fact, if the diagnosis can be made quickly and the genital edema responds to low volume PD, hernia repair may be performed without switching to hemodialysis [55]. For ventral or incisional hernias, open anterior repair with inversion of the hernia sac without disrupting it, and placing onlay mesh has been shown to have low recurrence and leak rates in adults [56,57]. If the peritoneum is entered, it is recommended to close the peritoneum in a watertight manner [58].…”
