Anterior Segment Optical Coherence Tomography for Transepithelial Phototherapeutic Keratectomy in Central Corneal Stromal Scarring

Abstract: Transepithelial PTK guided by ASOCT can allow for the precise and accurate removal of central corneal stromal opacities, resulting in a favorable refractive outcome.

“…The thin graft material was transparent enough to allow unobstructed imaging of underlying structures and the measurement of their dimensions. These observations were similar to those made in humans where OCT is used frequently in the operative scope of corneal surgery to aid in choice of treatment strategy . Currently, more data are needed to support this kind of protocol, but we believe that increasingly widespread use of OCT will improve treatment strategies in the future.…”

“…Anterior segment spectral domain OCT (SD‐OCT) has been the subject of more than 500 scientific articles over the last 5 years. The application of SD‐OCT for medical and surgical purposes has been specifically described for the structural analysis of tear meniscus, normal and pathological cornea, and iridocorneal angle, as well as evaluation of the anterior chamber, iris, and lens . OCT has also been compared with slit‐lamp examination and with Scheimpflug imaging for corneal endothelial evaluation …”

“…14,19,20 Anterior segment optical coherence tomography can also be used as a tool to measure the depth of haze when considering the type of corrective surgical management. [21][22][23] Superficial haze could be treated with phototherapeutic keratectomy (PTK) and superficial anterior lamellar keratoplasty, whereas deeper haze and scaring may require deep anterior lamellar keratoplasty.…”

“…Other groups are working on using synthetic masking agents, with an ablation rate similar to that for stroma, so that further smoothing can be performed after removal of the epithelium. [36][37][38][39][40] Transepithelial PTK treatments are also used in the treatment of corneal conditions such as recurrent erosion, 41,42 corneal dystrophies, 42,43 and scarring, 44,45 as well as for reducing the cone in keratoconus combined with cross-linking. 46,47 specific Developments: postoperative corneal ectasia…”
