Anomalous pulmonary vein detected using three-dimensional computed tomography in a patient with lung cancer undergoing thoracoscopic lobectomy

Abstract: Few clinicians are familiar with the anatomy of anomalous pulmonary veins, and studies reporting patients who required right lower lobectomy for lung cancer and who had anomalies of the middle and lower pulmonary veins are even rarer. This report describes the case of a lung cancer patient who had an anomalous lateral part of the middle lobe vein (V4) draining into the right inferior pulmonary vein, which was confirmed by three-dimensional 64-row multidetector computed axial tomography (3D-MDCT) angiography. “…Singular cases of drainage from the middle lobe to the superior and inferior pulmonary veins are also of interest; in such cases, the veins run from segment 4R down to the inferior pulmonary vein and from segment 5R to the superior pulmonary vein circuit. This phenomenon was estimated by one author to occur in approximately 3% of cases [11].…”

“…6) Cases of middle lobe veins draining into the right inferior pulmonary vein have also been reported. [7][8][9] Cronin et al analyzed 200 patients. 10) Among these patients, 12% had three atrial ostia in the right vein and in 9% of the patients the middle lobe vein drained into the left atrium, and 6.5% of those patients had a single left pulmonary vein ostium.…”

“…These high resolution scans, often available with three dimensional reconstruction, can provide valuable opportunities to identify standard and anomalous vascular variations prior to surgery in greater than 95% of cases (2)(3)(4)(5)(6). Imaging may also identify calcification of lymph nodes, which may predict a conversion rate of greater than 30% when hilar or central in location (3).…”
