Background: Congenital anamolies of extrahepatic biliary apparatus and pancreas have long been recognized and are of clinical importance because when present may surprise the surgeon during surgery and lead to iatrogenic injuries. Surgeries on extra-hepatic biliary apparatus and pancreas are regularly performed throughout the world. Thus insight into the normal anatomy and congenital variations will reduce complication and definitely improve outcome.Methods: Study was conducted in department of surgery GMC Bhopal and dissection was carried out in Department of Forensic Medicine on 100 cadavers with approval from ethical committee.Results: In 100 cases 70 were male and 30 female. The most common variation in extra hepatic biliary apparatus was short cystic duct was found in 6% cases then formation of common hepatic duct by union of right hepatic duct and left hepatic duct was intrahepatic in 3% cases. There was low insertion of cystic duct with common hepatic duct in 1% case. Cystic artery originating from left hepatic artery in 1% case, in 1% case cystic artery was anterior to common hepatic duct. In Pancreas anterior arterial arcade was absent in 2% cases and its origin varied in 2% case. Posterior pancreatic arcade absent in 1% cases and variation in origin was present in 1% case. The variation in pancreatic duct course was present in 22% cases.Conclusions: Thus significant variation was seen and it could definitely be helpful to hepatobiliary, laproscopic surgeons, radiologist and will further contribute to literature on variation of extrahepatic biliary apparatus and pancreas and its related vessels.

BACKGROUNDThe study of angle of union of biliary ducts is of great importance to the hepatobiliary surgeons. The union of cystic and common hepatic duct can be angular, spiral or parallel. The site of union can be average, high or low. MATERIALS AND METHODSStudy was conducted on 250 specimens (Cadaveric, Autopsy and Foetal) in Government Medical College, Thiruvananthapuram, for 2 years. Site and type of union was observed. RESULTSType 1 or angular union of ducts was most common. Low union of ducts was seen in majority. CONCLUSIONKnowledge of anatomy of biliary ducts is important for hepatobiliary surgeons, as the variations are very common and can cause complications during surgery.

Estudo anatômico das variações de posição da junção dos ductos INTRODUÇÃOA anatomia da árvore biliar extra-hepática é de grande relevância na cirurgia biliar, principalmente no que concerne às variações das junções dos ductos cístico e hepático comum (C-H), que vão se configurando ao longo do desenvolvimento embriológico do indivíduo 1-8 . Definir e classificar as variações anatômicas da junção dos ductos cístico e hepático comum em fetos, analisando a freqüência, trajeto e relação entre eles. Métodos: Dissecaramse 33 fetos , no período de setembro de 1999 a julho de 2000, utilizando-se fotografias para registrar as junções cístico-hepáticas. As uniões foram classificadas como alta, média e baixa e, quanto ao curso, paralelo ou angular. Constatado o tipo de união, aferiu-se o comprimento dos ductos. Resultados: Visualizou-se a junção cístico-hepática em 93,9% dos fetos, sendo encontrada a inserção média em 45,2% delas, a inserção alta em 29% e a inserção baixa em 25,8%. Quanto ao curso ductal, a união aguda foi observada em 71% dos fetos, enquanto a paralela, em 29%. Quanto ao comprimento ductal, o ducto cístico variou de 4 -6 mm, o ducto hepático comum de 9 -13 mm, e o ducto colédoco de 5 -10 mm. Conclusão: Dentre as variações anatômicas, a inserção média foi a prevalente, seguida do curso angular, com comprimentos dos ductos cístico e hepático comum variando entre 6mm e 7 mm, respectivamente. Foi demonstrada uma freqüência significativa de inserção baixa cístico-hepático comum. DESCRITORES Ducto cístico. Ducto hepático.
