Care providers for older immigrants in Sweden find themselves in a paradox. Individuals and associations call for culturally sensitive elderly care. However, implementing this comes at the risk of over-culturizing needs and behaviours, drawing a negative picture of 'the problem of immigrants' that needs to be solved with special interventions. To find a balance in this paradox, we applied the welfare theory of health to grasp a new understanding of the phenomena and draw a holistic picture of a person's needs and resources available to achieve good ageing, reaching beyond the cultural paradox. Semi-structured qualitative interviews were conducted with older adults with immigrant backgrounds in Sweden. The interviews were analysed using content analysis. Combining welfare theory of health with immaterial capital theories offered a holistic theoretical approach to good ageing. This took its departure from the agency of older adults, mitigating the gap between their vital life goals and available resources to reach these goals. Although informants wanted caring interventions from close family, we identified distinct responses to mitigate the diminished trust older adults had in the capability of welfare institutions to provide adequate elderly care.

This study analysed the prevalence of, change and variation in paid informal care as a policy strategy within Swedish municipal eldercare (n = 165) in 2006 and 2016. The results showed that the share of municipalities with this policy has declined since 2006, with just over half of the municipalities offering paid informal care in 2016. Among these municipalities, the practical use of this policy was limited in comparison to ordinary home-care services. Efforts to explain why some municipalities were more likely to have a system of paid informal care than others, were only partially successful, only identifyinga few significant municipal factors that together explained a limited share of the variation among the municipalities. The results raise questions relating to the way these policies are understood/used by municipalities and how older people and their informal caregivers are affected by the way in which the question of paid informal care is handled at the local level. ABSTRAKTDen här studien analyserar förekomsten, förändringen och variationen av betald anhörigomsorg som en policy strategi inom kommunal äldreomsorg (n = 165) 2006 och 2016. Resultaten visade att andelen kommuner med denna typ av policy minskade efter 2006 och att betald anhörigomsorg endast erbjöds i lite mer än hälften av kommunerna 2016. Bland dessa kommuner var den praktiska användningen av betald anhörigvård oftast begränsad i förhållande till användningen av traditionella hemtjänst. Försök att identifiera vilka kommunala faktorer som gör kommuner mer benägna att att erbjuda denna typ av policy lyckades endast delvis och större delen av variansen mellan kommer förblev oförklarad. Sammantaget väcker resultaten flera frågor som relaterar till hur kommuner förstår och använder sig av betald anhörigvård, samt hur hjälpbehövande äldre och deras anhöriga påverkas av hur frågan om betald anhörigvård hanteras på den kommunala nivån.

A typology of family caregiving for older immigrants: perspectives from care receivers and care providers

Purpose As populations are ageing and the global average life expectancy is rising, the provision of care for older people is an increasingly salient issue. This paper aims to focus on family-provided care for older immigrants, examining how older immigrants and care providers experience and construct family caregiving. Design/methodology/approach Based on interviews with care recipients, family care providers, municipal staff and representatives for migrant organisations in Sweden, this study presents a typology of family caregiving for older immigrants. Findings The authors found three caregiving types, namely, solely family-provided care and a combination of family care and public care (predominantly one or the other). The decision to select family-provided or publicly-funded care depends on personal and institutional factors. Originality/value The paper makes three empirical contributions to the literature on care provision for older immigrants. Firstly, this study provides insights into the structural and personal factors that shape care-giving arrangements for older immigrants. Secondly, this study examines the perspectives of care recipients and care providers on family-provided care. Care expectations differ between both groups and sometimes result in intergenerational disagreement. Thirdly, in terms of institutional support, this study finds that the Swedish state’s notion of individual needs does not match the needs of immigrant elderly and their caregivers. The paper places the care types in a broader discussion about eldercare provision in the Swedish welfare state, which has experienced a decline in publicly funded care services and an increase in family caregiving in the past 30 years. In addition, it addresses questions of dignified ageing from a minority perspective.
