Anhedonia Predicts Poorer Recovery Among Youth With Selective Serotonin Reuptake Inhibitor Treatment–Resistant Depression

Abstract: Objective To identify symptom dimensions of depression that predict recovery among SSRI-treatment resistant adolescents undergoing second-step treatment. Method The Treatment of Resistant Depression in Adolescents (TORDIA) trial included 334 SSRI-treatment resistant youth randomized to a medication switch, or a medication switch plus CBT. This study examined five established symptom dimensions (Child Depression Rating Scale-Revised) at baseline as they predicted recovery over 24 weeks of acute and continuati… Show more

“…However, depression severity was somewhat lower in this sample (baseline CDRS-R mean = 53.2, SD 9.3) than in other trials such as TADS (mean = 60.0, SD 10.4) 54 or TORDIA (mean = 59.0, SD 10.0). 55 That our sample was slightly less depressed is not surprising, given that these youth were initially identified as depressed in primary care. The enduring CBT benefits observed in this study for major depression recovery may have been partially due to this lower baseline depression level.…”

“…Given anhedonia was previously reported as a valid predictor of poor response to antidepressant treatment (McMakin et al.,2012) and adversity during childhood was more frequently observed in patients with treatment‐resistant depression than patients with treatment‐sensitive depression (Tunnard et al., 2014). It could be naturally speculated that the depression induced by early life stress tends to exhibit treatment resistance, and this needs to be assessed in further studies.…”

“…Parent-child conflict has a bidirectional relationship with treatment outcome in adolescent treatment resistant depression [17]. Anhedonia in depressed adolescent's refractory to medication treatment predicted longer time to remission and fewer depression-free days [18]. Depression severity and subsyndromal manic symptoms predicted poor treatment response in this sample [19].…”
