A 57-year-old woman presented with cough, dyspnea, and recurrent pleural effusion. The patient had previously undergone thoracentesis for left-sided pleural effusion at another medical clinic. A chest computed tomographic (CT) scan revealed pleural effusion and mediastinal tumor in the left para-aortic location. The tumor was resected using video-assisted thoracoscopic surgery (VATS) and revealed as a benign non-infiltrating angiolipoma. This is the first case report of a non-infiltrating mediastinal angiolipoma not localized to the posterior mediastinum.
