Angioleiomyoma of the Extremities: Findings on Ultrasonography and Magnetic Resonance Imaging

Abstract: Angioleiomyoma of the extremities is usually a well-circumscribed oval mass with a homogeneous echo texture and occasionally hypoechoic protrusions on US and shows heterogeneous enhancement with occasionally enhancing structures on one or both ends of the mass on MRI. Therefore, it should be included in the differential diagnosis of a soft tissue mass that has protruding structures from one or both ends.

“…Demonstrated by the case findings, literature reports angioleiomyomas to consistently demonstrate spindle-shaped cells set in bundles with elongated cigar-shaped nuclei and surrounding vascular channels lined by regular endothelial cells [ 4 , 14 , 15 ]. Other reported histological findings include myxomatous or hyaline degeneration [ 4 , 14 , 15 ]. Current literature reports simple excision and primary closure to be the treatment of choice, with rare recurrence rates (approximately 0.36%) and minimal morbidity [ 3 ].…”

“…The MR pattern is nonspecific with variable enhancement on contrast-enhanced T1-weighted fat-suppressed sequence. However, enhancing vascular structures observed in at least 50% of the cases, on one or both ends of the mass, are suggestive of the diagnosis [80,81].…”

“…Kumar et al [27] mentioned that diffuse homogeneous enhancement of the mass lesion with a vessel converging on the superior pole of the mass lesion was shown on postcontrast fat suppressed T1-weighted images. Kang et al [28] described that slightly high signal intensity on T1-weighted image, a heterogeneous and high signal intensity on T2-weighted and STIR images, and heterogeneous enhancement on enhanced T1weighted image were shown. In addition, enhancing structures on one or both ends of the mass on MRI has been reported.…”
