Gynecologic Oncology has changed in the last few decades. An increasing proportion of patients is benefiting from long term survival although patients diagnosed with advanced disease suffer from a poor prognosis. Unfortunately, several recent studies are confirming that changing the combinations of "traditional" cytotoxic drugs is unlikely to obtain a real breakthrough in survival rates. Furthermore, there is discouraging data regarding consolidation therapies. It is, therefore, necessary to identify new target therapies with a minimal impact on quality of life. It is likely that new breakthroughs are only going to be achieved when changes in therapies are tailored to the entire natural history of the disease and on specific patient characteristic's. Since the discovery that tumor infiltrating lymphocytes represent an independent prognostic factor in ovarian cancer patients, several researchers have dedicated their attention to cancer immune response in order to identify prognostic factors and immunological targets. Analyses on immunophenotype at diagnosis and throughout the entire course of treatment are currently ongoing and are giving the new diagnostic, prognostic and therapeutic tools to the physicians. Furthermore new antigens and new vaccination strategies are being investigated. Encouraging data on selected patient populations have been observed recently. The objective of the present review is to define the immunology of women affected by gynecological malignancies and describe new immunological targets for prognostic and therapeutic use.

Vulvar carcinoma is a rare female genital neoplasia. Radical surgery, which has been the standard treatment approach, is often accompanied by considerable morbidity. To reduce the incidence of complications there has been a movement toward individualised therapy and less radical surgery. Associated with this, new tumour markers that could serve as prognostic indicators would be of considerable value to guide treatment decision. In this review, a brief update of molecular pathological markers of vulvar carcinomas is provided, and their impact as prognostic markers is addressed. p16, p21, p14, p27, cyclin A, cyclin D1, p53, vascular endothelial growth factor (VEGF), transforming growth factor alpha, HER-2 and epidermal growth factor receptor (EGFR) have been found to be important in the pathogenesis and/or progression of vulvar carcinomas. Furthermore, human papillomavirus, p16, p21, p14, p53, VEGF, CD44v3, CD44v6, CD44v4, CD44v9, CD44v10, HER-2, EGFR, matrix metalloproteinase-12, caspase 3, Bcl-2 and nm23-H1 have been correlated to clinical outcome of patients with vulvar carcinomas. However, due to the relative small number of studies reported for each molecular pathological marker, and the relative small number of vulvar carcinomas included and the lack of multivariate analysis in the majority of these studies, no conclusion regarding the prognostic value of these markers can be drawn. Therefore, the investigated markers have not yet earned a place in standard clinical diagnostics or treatment, and further studies are needed to clarify the clinical value of these markers.

ZusammenfassungDas Vulvakarzinom ist ein seltener Genitaltumor der Frau. Der Häufigkeitsgipfel liegt im Alter zwischen 70 und 79 Jahren mit einer langsamen Tendenz zur altersmäßigen Vorverlagerung. In über 90% handelt es sich um ein Plattenepithelkarzinom. Die zugehörige intraepitheliale Präneoplasie VIN tritt am häufigsten bei Frauen unter 35 Jahren auf. Heute geht man davon aus, dass 2 Typen unterschieden werden können: eine durch das humane Papillomavirus (HPV) induzierte, eher bei jüngeren Frauen auftretende Neoplasie und eine HPV-unabhängige Neoplasie eher im höheren Alter. Bereits die VIN sollte chirurgisch entfernt werden. Die invasiven Karzinome können im Stadium I und II durch eine lokale weite Exzision im Gesunden reseziert werden, die inguinofemoralen Lymphknoten werden ab einer Eindringtiefe von 1,1 mm reseziert. Bei größeren Tumoren wird eine Vulvektomie mit beidseitiger Lymphonodektomie erforderlich. Bei weit fortgeschrittenen Tumoren kommen multimodale Therapieplanungen zum Einsatz, dabei kann eine primäre Radiochemotherapie einer Salvage-Operation vorangestellt werden. It appears mostly in older women aged 70-79 years with a slow tendency toward younger age. More than 90% of the tumors show a squamous differentiation. The correspondent preneoplasia is VIN. This lesion occurs in women mostly younger than 35 years. Experts assume that vulvar cancer appears in two different types: HPV-induced type in younger women and non-HPV-dependent type in older women. The preneoplasia VIN already should be treated by resection or destruction. Invasive carcinomas stage I or II can be treated by wide local excision. The inguinofemoral lymph nodes should be resected if invasion exceeds 1 mm in depth. In larger primary tumors, vulvectomy with bilateral inguinofemoral node dissection is indicated. In advanced tumor stages, multimodal concepts are applied: primary radiochemotherapy may precede a salvage operation.
