Zusammenfassung Hintergrund Die adäquate Versorgung von Traumapatienten nach Verkehrsunfällen (VU) setzt eine frühe und bedarfsgerechte Disposition geeigneter Rettungsmittel sowie eine dem erwarteten Verletzungsmuster gerecht werdende Schockraumbehandlung voraus. Daher sind validierte Diskriminanten und eine möglichst hohe Vorhersagegüte zur Alarmierung aller Mitglieder der Rettungskette notwendig. In der aktuellen Version der S3-Leitlinie Polytrauma/Schwerverletzten-Behandlung werden unfallbezogene Schockraumalarmierungskriterien genannt, welche in dieser Arbeit anhand der GIDAS-Datenbank für Verkehrsunfälle reevaluiert werden. Methodik In der vorliegenden deskriptiven, retrospektiven Registerstudie wurden 42.310 Verkehrsunfälle mit 57.837 Pkw-Insassen anhand der GIDAS-Datenbank (01/1999–06/2021) analysiert und Parameter zur Vorhersage von schweren Verletzungen (AIS 3+) erhoben. Ergebnisse Die Parameter „Intrusion von mehr als 50–70 cm“ (positiver prädiktiver Wert [PPW] 48–59 %), „Geschwindigkeitsänderung > 30 km/h“ (PPW 10 %) und „Fußgänger‑/Zweiradkollision“ (PPW 8 %) zeigten eine geringe Vorhersagegüte oder sind prähospital nicht valide erhebbar. „Tod eines Insassen“ (PPW 45 %) und „Ejektion aus dem Fahrzeug“ (PPW 57 % für herausgeschleuderte Person, PPW 28 % für Mitinsassen) zeigten hingegen eine gute Vorhersagegüte. Weiterhin zeigten die Parameter „Fahrzeugdeformierung“ ab 130 cm (Frontanprall, PPW 33 %) und 100 cm (Seitenanprall, PPW 24 %) eine gute Vorhersagegüte und könnten zur Verbesserung der adäquaten Schockraumalarmierung dienen. Diskussion In unserer Studie konnten Diskriminanten mit guter und schlechter Vorhersagegüte einer schweren Verletzung nachgewiesen werden. Diese sollten Grundlage für die Aktivierung eines Schockraumteams B (Teilalarm) sein und Einzug in die kommende Version der S3-Leitlinie Polytrauma/Schwerverletzten-Behandlung finden. Zudem sollten weitere Parameter überprüft und im Rahmen von Abfragemodellen den Rettungsleitstellendisponenten zur Verfügung gestellt werden.
