Aneurysmal (angiomatoid) fibrous histiocytoma

Abstract: We report a patient with aneurysmal (angiomatoid) fibrous histiocytoma manifested clinically as a single firm subcutaneous nodule, diagnosed as an epithelial cyst. Histologically, the growth showed massive histiocytic proliferation and areas with a vascular component. The lesion was histologically mistaken for Kaposi's sarcoma.

“…Aneurysmal dermatofibroma is commonly confused with benign and malignant vascular tumours including angiosarcoma, Kaposi sarcoma and spindle cell haemangioendothelioma, and with angiomatoid malignant fibrous histiocytoma. [7][8][9][14][15][16][17] Dermoscopy is a noninvasive, in vivo method for the diagnosis of pigmented and nonpigmented skin lesions. There have been many investigations concerning melanocytic lesions, especially malignant melanoma.…”

“…Differential diagnoses comprise angiomatoid fibrous histiocytoma, spindle cell hemangioma, malignant melanoma, Kaposi sarcoma, and angiosarcoma [22,23,[25][26][27]. The genetic information on aneurysmal BFH is restricted to a single case, cytogenetic analysis of which detected the translocation t(12;19)(p12;q13) as the sole chromosomal abnormality [13].…”

“…Aneurysmal fibrous histiocytoma has the potential for misdiagnosis and the differentials could include malignant melanoma, Kaposi's sarcoma (12), spindle cell hemangioma, angiosarcoma, and angiomatoid fibrous histiocytoma. The underlying common theme of the aforementioned misdiagnosis entities stems from the fact that there are large blood pools with extravasation of red cells in a spindly and cellular tumor.…”
