Anesthetic Management of the Patient With Dilated Cardiomyopathy Undergoing Pulmonary Resection Surgery

Abstract: Interactions between the cardiovascular and respiratory systems are complex and profound. General anesthesia, muscle relaxation, and positive-pressure ventilation all impose physiological effects on cardiovascular function. In patients presenting for pulmonary resection, additional effects resulting from positioning, 1-lung ventilation, surgical procedures, and contraction of the pulmonary vascular bed may impose an additional physiological burden. For most patients with adequate pulmonary and cardiovascular r… Show more

“…16.8% of exclusions were due to multimorbidity or another cause of limited life expectancy. Among multimorbid patients, a severely reduced LVEF may lead to an unacceptably high interventional risk [22,23], especially in the context of deep conscious sedation, which is needed during the procedure [24]. Almost one half of the patients excluded for multimorbidity (44.4%) had a severely reduced LVEF.…”

“…Also, pulmonary capillary bed reduction and insufficiency of pulmonary ventilation function after lung resection would result in decreasing of lung ventilation/perfusion ratio and increasing of pulmonary capillary blood flow (28). Cardiac function would also significantly be reduced (29). The above factors played important roles in the induction of arrhythmia.…”
