Anesthesia for patients with tracheal bronchus

Abstract: Tracheal bronchus is a congenital anatomical variant of patients with accessory bronchus in the right upper lobe deriving directly from the supracarinal trachea. Pre-operative consultation with the anesthesiologist is important for patient safety and can avoid adverse effects induced by endotracheal intubation. In this report, we described a case of tracheal bronchus in which general anesthesia was performed for video-assisted thoracoscopic surgery. We discussed some of the issues surrounding to complications … Show more

“…Obstruction of TB due to distal placement of endotracheal tube is a conceivable problem. [17,18] Patients with TB incidentally detected on CT images should be informed to prevent probable anesthesia complications in future. 'One lung ventilation' is an anesthesia procedure commonly performed in thoracic surgery and video assisted thoracoscopic surgery.…”

“…[6] This study will contribute to literature and will serve data of incidence to anesthesiologists in our country. Lai et al [18] suggest that TB is an asymptomatic entity which is incidentally diagnosed on CT. They also argue that TB causes recurrent infections and respiratory distress especially in children.…”

“…In the setting of TB, it is worth mentioning that endotracheal intubation merits special consideration. Misplacement of the endotracheal tube can occlude the TB lumen, resulting in atelectasis, hypoxia, or even respiratory insufficiency [14,15]. Hence, knowledge of TB anatomy is essential perioperatively to take the necessary precautionary measures and avoid complications [2,16].The diagnosis of TB can be achieved through high-resolution CT scans of the chest or by bronchoscopy [5,7].…”

“…Tracheal bronchus was first described in 1785. 2 The right upper lobe bronchus normally originates approximately 1-3 cm distal to the carina. In the current patient, it arose from the right lateral wall of the trachea at a distance of <1 cm proximal to the carina.…”
