Anesthesia and tau pathology

Abstract: Alzheimer’s disease (AD) is the most common form of dementia and remains a growing worldwide health problem. As life expectancy continues to increase, the number of AD patients presenting for surgery and anesthesia will steadily rise. The etiology of sporadic AD is thought to be multifactorial, with environmental, biological and genetic factors interacting together to influence AD pathogenesis. Recent reports suggest that general anesthetics may be such a factor and may contribute to the development and exacer… Show more

“…Tau phosphorylation may contribute to the cognitive impairment observed in rodents following anesthesia and surgery [reviewed in 33 ]. Anesthesia with 3% sevoflurane two hours daily for three days (multiple exposures of sevoflurane) in P6 WT mice has been reported to induce cognitive impairment in the mice 9 .…”

“…Tau phosphorylation is regulated by several protein kinases such as glycogen synthase kinase 3β (GSK3β) 2427 , cyclin-dependent kinase 5 (CDK5) 25,28 , cJun n-terminal kinase (JNK), 29,30 and extracellular signal-regulated kinase (ERK) 31 [reviewed in 32,33 ]. The expression of the kinases peaked postnatally at days 8–11 and then returned to low level after 5 weeks 34 .…”

“…A principle side effect of vGA exposure during surgical procedures is signifi cant drop in body temperature, hypothermia. The enzymatic activity of PPA2A is directly tied to temperature, thus the lowered thermostat has been found to inhibit the dephosphorylation of tau by indirect means, abnormally increasing hyperphosphorylated tau [27,32,33]. SEVO induces increased p-tau/t-tau ratios via inhibition of mammalian target of rapamycin (mTOR); a pathway mediated by GSK-3, indicating vGAs can directly infl uence intracellular kinase activities [34,35].…”

“…Propagation can occur by direct seeding of tau oligomers into the lipid rafts of cell membranes, thus increasing cell permeability and allowing access for the spread of larger secreted NFTs. 16 After NFTs propagate, the post-translational modifications become finalized, and behavioral and motor symptoms begin to surface in patients.…”
