ANCA-Associated Vasculitides and Hematologic Malignancies: Lessons from the Past and Future Perspectives

Abstract: The purpose of this paper is to collect and summarize all evidences relating to an association between ANCA-associated vasculitides (AAVs) and hematologic malignancies, in the form of either a paraneoplastic vasculitis or leukemias and lymphomas developing on a preexisting vasculitis. Additionally, the role of cyclophosphamide in vasculitis treatment has been assessed and compared to rituximab. Paraneoplastic AAV seems to be an uncommon presentation of hemopathies. Hematologic malignancy risk in AAV is more li… Show more

“…AAV as a paraneoplastic phenomenon seems to be an uncommon presentation of hematological malignancies. However, their coexistence was previously observed, e.g., in a case where initial presentation with cutaneous vasculitis led to the diagnosis of hairy cell leukemia [21,22]. Pulmonary malignancies are not usually reported at AAV diagnosis in patients with pulmonary symptoms.…”

“… 17 Why ANCA-associated glomerulonephritis can occur in a paraneoplastic manner is poorly understood; however, a dysregulated inflammatory-cell response, elevation of various cytokines, and the cross-reactive nature between cancer cells and vascular endothelium in terms of antigen complementarity have been proposed as candidate mechanisms leading to this pathology. 14 , 16 , 17 , 19 Alternatively, or in addition, such a link may result from patient exposure to an as-yet-unidentified common risk factor shared by these 2 disease states. 17 …”
