Anatomy of the Corrugator Supercilii Muscle: Part I. Corrugator Topography

Abstract: The dimensions of the corrugator supercilii muscle are more extensive than previously described and can be easily delineated using fixed bony landmarks. These data may prove beneficial in performing safe, complete, and symmetric corrugator supercilii muscle resection for forehead rejuvenation and for effective decompression of the supraorbital nerve and supratrochlear nerve branches in the surgical treatment of migraine headaches.

“…We have designed surgical techniques and investigated why surgery may play a role in the amelioration of this disabling condition. [13][14][15][16] There are still many unanswered questions, and our team continues its efforts to find more logical pathophysiological reasons for the surgical success that our patients have been enjoying.…”

“…The research team has been investigating its efficacy over the last 10 years. 9 -12 Our retrospective, 9 pilot prospective, 10 and anatomical studies [13][14][15][16] have all provided a foundation for this more comprehensive investigation to assess the effectiveness of this technique over a 5-year follow-up period. Our in-depth, previously published study of the surgical treatment of the four most common migraine trigger sites 11 reported the outcome of the treatment at 1-year follow-up and compared the treatment group with controls who did not undergo surgery.…”

“…To appreciate the differences and advantages in BTX-A injection techniques between the two specialties, it is important to understand the different targets of injection, the trigger point and nerve compression hypothesis, mechanism of action of BTX-A in the treatment of migraines, and the anatomy of various muscles and nerves has only recently been elucidated by studies done between Cleveland and Dallas [6,7,[18][19][20]. In this chapter, we discuss the PREEMPT injection paradigm and the Anatomical Regional Targeted (ART) BTX-A paradigm.…”

“…1,2 The intimate anatomy of the branching patterns of the supraorbital nerve within the corrugator supercilii has further been evaluated to guide a more effective deactivation of this trigger point. 3 Knowledge of this anatomy has led to frontal decompression reducing migraine headaches by half in 83.7 percent versus 57.7 percent of a sham surgery cohort, and completely eliminated in 57.1 percent of the surgical arm and in 3.8 percent of sham operations.…”
