Moderate Heat-Assisted Gene Electrotransfer for Cutaneous Delivery of a DNA Vaccine Against Hepatitis B Virus

An estimated 350 million people are living with chronic Hepatitis B virus (HBV) worldwide. Preventative HBV vaccination in infants has reduced the disease burden; however, insufficient immunization programs and access obstacles leave vulnerable populations at risk for infection in endemic regions. Gene electrotransfer (GET) using a noninvasive multielectrode array (MEA) provides an alternative platform for DNA vaccination in the skin. DNA vaccines are nonlive and nonreplicating and temperature stable unlike their counterparts. In addition, their simple engineering allows them to be manufactured quickly at a low cost. In the current work, we present the combination of GET and moderate heating for delivery of a DNA vaccine against HBV. Our laboratory has previously shown the synergy between moderate tissue preheating at 43°C and GET with the MEA as a means to reduce both the applied voltage and pulse number to achieve similar if not higher gene expression than GET alone. In this study, we expand upon this work, by optimizing the plasmid dose to achieve the highest level of expression. Using the reporter gene luciferase, we found that an intradermal injection of 100 lL at 1 mg/mL induced the highest expression levels across all tested GET conditions. We then evaluated our moderate heat-assisted GET platform for the intradermal delivery of a plasmid encoding Hepatitis B surface antigen (pHBsAg) via a prime and prime plus boost vaccination protocol. At 18 weeks, following the prime plus boost protocol, we observed that a high-voltage low-pulse GET condition with moderate heating (45 V 36 p+heat) generated antibodies against Hepatitis B surface antigen (HBsAb) at peak measuring 230-fold over injection of plasmid DNA alone with moderate heating. HBsAbs remained robust over the 30-week observation period. These data suggest that moderate heatassisted GET has the potential to induce strong immune responses, an attractive feature for development of an alternative vaccine delivery platform.

Objective: This study was performed experimentally and single-blind randomized controlled trial to compare the effects of Helfer Skin Tap technique and Shot Blocker application on pain in the intramuscular injectıon application to the deltoid muscle. Methods:The study was designed as a prospective, single-blind randomized controlled trial. The research was conducted in accordance with the Consolidated Standards of Reporting Trials (CONSORT) Checklist guide. The study was conducted in a family health center in Sivas Province between 05.10.2020-30.12.2020. Individuals who received Hepatitis B vaccine injection were included in the sample of the study. All intramuscular injections were made by the same nurse, and the approaches to reducing pain were made by the researcher. A total of 120 patients including the Helfer Skin Tap Technique (n=40), the standard application (n=40) and the ShotBlocker technique (n=40) were included. Mann Whitney U and Kruskal Wallis tests were used to evaluate the data.Results: In our study, 75% (n=30) of the Helfer Skin Tap group, 82.50% (n=33) of the standard administration group and 75% (n=30) of the ShotBlocker group reported that they had no fear of injection. When the distribution of post-injection pain scores of the individuals according to different injection methods was examined, the difference between the average pain scores of the different methods used to provide pain control was found to be significant and statistically significant (p=0.001). Conclusion:In line with the findings of our study, it was concluded that the use of ShotBlocker in intramuscular injection application of individuals was more effective in reducing pain due to injection compared to Helfer Skin Tap and standard application, and the Helfer Skin Tap technique was more successful in pain control than standard practice. All health professionals', especially nurses', awareness of the effectiveness of different methods in reducing pain during IM injection should be raised. To ensure pain control during IM injection, Shotblocker and Helfer Skın Tap Technıque should be preferred since they are easy to apply and cost effective,

The entire globe is struggling with the COVID-19 pandemic since March 11, 2020. There is still a large number of infected patients and death, and there is no proven treatment for the infection yet. This has led to the race in vaccine development to protect people from COVID-19 infection. As of February 3, 2021, there were 289 experimental COVID -19 vaccines in development, 66 of which were in clinical trials with different phases, and 20 of them were in phase 3. This study aims to evaluate 15 important vaccines based on criteria such as the dose number, dosing schedule, storage advantages, efficacy, and side effect. In this evaluation, we use the fuzzy PROMETHEE approach, which is an important Multi-Criteria Decision Making (MCDM) technique. The importance level of the criteria is determined based on expert opinion. The result shows that the EpiVacCorona vaccine is the most effective vaccine to prevent COVID-19 infections based on the selected criteria and the importance level of each criterion. The result obtained may change based on individual or expert’s priorities. Due to the use of different criteria for the ranking and different weightings of the criteria, the ranking result may differ. This study also shows the strengths and weaknesses of the selected vaccines and the applicability of the MCDM technique for the evaluation of COVID -19 vaccines.
