INTRODUCTION: Anatomic variations of cerebral arteries may influence the development of aneurysms. In this study our aim was to determine vascular anatomical variations of the anterior circulation of the brain in patients with and without cerebral aneurysms and also the relationship between vascular variations. METHODS: The cerebral digital subtraction angiography data of 438 patients were investigated retrospectively. Anatomical vascular variations of MCA and ACA-AcomA complex were investigated separately in patients with and without aneurysm. RESULTS: Most of our patients with cerebral aneurysms had SAH (n: 68, 80%). Although normal ACA AcomA complex and MCA vascular anatomy was less frequently seen in SAH, there was no statistical significance. ACA AcomA vascular anatomy was not significantly different between patients with and without cerebral aneurysm, however patients with cerebral aneurysm had statistically higher MCA vascular anatomical variations (p: 0.03). DISCUSSION and CONCLUSION: ACA-AcomA vascular anatomy was not significantly different between patients with and without cerebral aneurysm. However MCA vascular anatomical variations were more commonly seen in patients with cerebral aneurysm. Variations of MCA may increase the risk of aneurysm formation and therefore have an important role in the planning of aneurysm treatment.

SUMMARY:One of the most common causes of stroke is rupture of aneurysms whose approach requires knowledge of anatomical variants. The aim of this study was to determine the prevalence of anatomical variants of the anterior cerebral artery (ACA) and the anterior communicating artery (AComA) by 3D computed tomography angiography (3D CTA) in Mexican individuals. A retrospective, observational, cross-sectional descriptive study of 283 patients, independent of sex or age, in which morphometric measurements of cerebral vessels were evaluated using contrasted 3D CTA from a period of two years was performed. We found at least one "atypical" variant in a third of the study population (33.93 %). The most common "atypical" variant was the absence of the AComA (14.1 %). A significant association between the hypoplastic variant of the right A1 segment and hypoplasia of the left A1 and the right A2 was found, while hypoplasia of the left A1 was associated with hypoplasia of the right A2. There is a difference in the prevalence of anatomical variants of the ACA-AComA complex in Mexican population in relation to other populations. The typical variant is the most prevalent form in the study population. However, the presence of atypical variants represents an important number that should be taken into account in clinical and surgical procedures.

Background. The anterior communicating artery (ACoA) complex consists of the ACoA, the pre-and post-communicating segments of the anterior cerebral artery, and the recurrent artery of Heubner. It is the most common site for anatomical variations in the circle of Willis. Such variations can mimic intracranial aneurysms. Case description. A 30-year-old female presented with recurrent episodes of extreme headache and bilateral tinnitus. A brain computed tomography (CT) scan showed no significant lesions, while her CT-angiography (CTA) showed an enlarged vascular lesion at the ACoA, raising the suspicion for an ACoA aneurysm. A repeated CTA revealed a rare anatomical variation with a pattern of cross dominance in the ACoA complex; the left A1 and right A2 were dominant-enlarged, resulting in an enlargement of the ACoA. The presence of an ACoA aneurysm was hence excluded and the patient was managed conservatively. At 6-month follow-up, CTA showed no new findings. Conclusion. ACoA enlargement can result from unequal hemodynamics around the ACoA complex, which may be mistaken for an aneurysm. A thorough study of the imaging data is of pivotal importance and may change the management strategy.
