Anatomical Suitability of the Aortic Arch Arteries for a 3-Inner-Branch Arch Endograft

Abstract: Purpose: To analyze aortic arch anatomy of patients who were already treated with a 2-inner-branch arch endograft (2-IBAE) in order to assess the anatomical suitability of the supra-aortic arteries as target vessels for a 3-IBAE. Materials and Methods: Three different configurations of the Cook Zenith Arch endograft were designed with distances of 110 mm (model 1), 90 mm (model 2), and 70 mm (model 3) between the orifices of the first and third inner branches. Preoperative measurements of the aortic arch anato… Show more

“…Currently, although the Cook Arch Endograft has three inner branches with a retrograde branch for the left subclavian artery 6,7 which allow to work with caudal oriented thoracoabdominal branches, the conduit here described can be used as an alternative approach whenever the aortic arch is anatomically challenging.…”

“…The anatomic suitability criteria have previously been reported. 16,17 These included iliofemoral access suitable for a 22F to 24F delivery system, a length from the sinotubular junction to the IA of $50 mm, a proximal sealing zone length of $40 mm for the native aorta and $20 mm for a surgical graft with a diameter of #38 mm, suitable sealing zones for all three supra-aortic trunks, and the absence of excessive kinks or atherosclerotic debris in the aortic arch. 15 For the LSA, a diameter <12 mm with distance of $20 mm from the ostium of the vertebral artery to the origin of the LSA has been recommended.…”

“…Dopo il rilascio dell'endoprotesi e il bridging stenting dei due vasi sovra-aortici prossimali, dall'accesso femorale omolaterale è possibile far avanzare all'interno del branch pre-incannulato un introduttore dedicato e procedere in via parallela all'incannulamento e stenting dell'ASS. Recentemente, Spanos et al 11 hanno riportato uno studio sulla fattibilità anatomica della configurazione di endoprotesi dell'arco…”
