Background The anatomy of the clavicle is specific and varied in reference to its topography and shape. These anatomic characteristics play an important role in the open treatment of clavicle fractures. The complex and variable topography creates challenges for implant placement, contouring, and position. Hardware prominence and irritation does influence the decision for secondary surgical intervention. Methods Computerized tomographic scans of 350 adult clavicles with the corresponding patients' metadata were acquired and digitized. Morphologic parameters determining the shape of the clavicle were defined and computed for each digitized bone. The extracted morphologic parameters were correlated with patient metadata to analyze the relationship between morphologic variability and patient characteristics. Results The morphologic parameters defining the shape, that is, the radius of the medial and lateral curves, the apparent clavicle height and width, and the clavicle bow position, correlate with the clavicle length. The clavicle length correlates with the patients' height. Gender differences in shape and form were dependent and related to individual height distribution and clavicle length. Asian populations showed a similarly predictable, but shifted, correlation between shape and clavicle length. Conclusion This anatomic analysis shows that the clavicle shape can be predicted through the clavicle length and patients' stature. Smaller patients have shorter and more curved clavicles, whereas taller patients have longer and less curved clavicles. This correlation will aid surgeons in fracture reduction, implant curvature selection, and in optimal adaptation of clavicle implants, and represents the basis for anatomically accurate solutions for clavicle osteosynthesis.

Biodegradable materials are used in two key sectors of orthopaedics – to fabricate bone fixators and scaffolds for bone tissue regeneration. In case of osteosynthesis, fixators made from biodegradable materials disappear from the body after a certain time. So, a necessity of a one more operation for their removal is excluded. In the present study, the acromioclavicular joint osteosynthesis plates made of magnesium alloy (WE43), titanium alloy (Ti-6Al-7Nb) and stainless steel (316L) are compared utilizing the finite element analysis. The research showed that stresses in the magnesium alloy plate were lower, compared to the titanium alloy plate or the stainless steel plate. However, the tensile strength of magnesium is over 2 times lower, as compared to stainless steel and 5 times lower, than titanium alloys. Magnesium alloy is not suitable for manufacturing plates with low thickness (2 and 2.5 mm), because the stresses generated in them exceed the yield strength of the material.

Morphological clavicular variation has been studied extensively by orthopaedic surgeons, anatomic and forensic experts to explain developmental, gender and age-related differences. The design of fixation devices for displaced clavicular fracture management depends largely on anatomic characteristics of clavicle. Eighty-one unpaired clavicles of unknown sex were studied, 42 clavicles were of right side and 39 clavicles of left side. All the clavicles were collected from adult cadavers which were dissected in Departments of Anatomy at Muhimbili and Herbert Kairuki Memorial Universities. The length of clavicle was measured by a vernier calliper; the middle point of this length was taken as the point where midclavicular circumference was measured with the help of a measuring thread and the angle of curvature of clavicle was measured by using protractors. The average lengths of the left and right clavicles were 15.23±1.12 cm and 15.43±1.01cm respectively. The average medial angle of curvature of left clavicle was 155.33° ± 4.39°and that of right clavicle was 153.40° ± 3.96°. The mean total angle of curvature of left clavicle was 293.54˚± 9.55°and the average total angle of curvature of right clavicle was 290.05±8.94°. The average midclavicular circumference of left clavicle was 3.88cm ± 0.33cm and that of right clavicle was 3.94cm±0.33cm. The right clavicle was longer than the left clavicle, the average medial angle of curvature of left clavicle was greater than medial angle of right clavicle, the average lateral angle of curvature of left clavicle was more than the average lateral angle of curvature of right clavicle and the mean of midclavicular circumference of right clavicle was greater than that of left clavicle.
