Analysis of zonular-free zone and lens size in relation to vial length of eye with age

Abstract: When choosing an intraocular lens, one should consider the patient's age and the A-P globe length. Six eyes (5%) showed unusual anterior insertion of zonular attachment, which reminds us to be prudent with a large paracentral capsulorhexis.

“…2 Studies show that capsular bag diameter tends to correlate with an increase in AL. [15][16][17] Thus, early rotation is more likely to occur in highly myopic eyes. Patel et al 6 used serial digital photography to compare rotation between plate-haptic IOLs and loop-haptic IOLs.…”

“…The series is too small to confirm whether certain patient groups are at risk, although the 2 cases with phimosis had retinitis pigmentosa, which is associated with aggressive capsule fibrosis, and axial myopia in which there is a relatively large capsular bag. 6,7 The phenomenon of delayed hyperopic shift following cataract surgery has been described after implantation of a single-piece Collamer plate-haptic IOL (CC420BF, Staar Surgical Co.) with a minimum incidence of 0.02%. 8,9 With this IOL, the mean hyperopic shift in affected cases was 1.81 D (range 0.25 to 3.75 D) occurring between 1 week and 11 months after surgery.…”
