Analysis of the early postoperative serum carcinoembryonic antigen time-course as a prognostic tool for bronchogenic carcinoma

Abstract: Although PROD is more sensitive than PLAT, both parameters appear to be useful as prognostic tools for predicting early recurrence after resection of lung carcinoma. This is probably because they represent the number of residual tumor cells immediately after surgery.

“…After a short term increase immediately after the intervention due to marker release from operatively damaged normal and tumor tissue, the decline depends on both biological marker half-life and residual tumor cells [3,37,38]. After curative tumor resection, the levels of CYFRA 21-1, TPA, and SCC -which have a half life of about 1.5 to 3 hoursare expected to decrease rapidly reaching the range of healthy persons within one to two days.…”

“…After curative tumor resection, the levels of CYFRA 21-1, TPA, and SCC -which have a half life of about 1.5 to 3 hoursare expected to decrease rapidly reaching the range of healthy persons within one to two days. CEA decrease occurs with some delay depending on the initial marker level and a longer half life of one to four days [3,5,24,31,37,38]. It has to be emphasized that renal or liver dysfunction may prolong the half life of tumor markers considerably [19,21,29].…”

“…Other studies have also shown the prognostic value of squamous cell carcinoma (SCC) antigen (9–11), carcinoembryonic antigen (CEA) (11–13), carbohydrate antigen 125 (CA125) (11,14) and even neuron-specific enolase (NSE) (15) in non-small cell lung cancer. It has also been shown that the most effective way of evaluating prognosis is dynamically, by comparing pre- and post-therapy results (16,17). …”

“…We previously reported that these two parameters provided information about the prognosis of patients after surgery. 11,12) High postoperative CEA level at the plateau, as well as large postoperative CEA production, indicate the presence of remaining CEA-producing cancer cells. These two parameters may thus be able to predict tumor recurrence.…”
