Analysis of systemic sclerosis in twins reveals low concordance for disease and high concordance for the presence of antinuclear antibodies

Abstract: Objective. To examine concordance for systemic sclerosis (SSc) in monozygotic (MZ) and dizygotic (DZ) twins.Methods. MZ and DZ twins were recruited nationwide. Zygosity was confirmed by DNA fingerprint analysis. The presence of antinuclear antibodies (ANAs) was determined using indirect immunofluorescence with HEp-2 cells as substrate. Identification of SScassociated serum autoantibodies was performed by immunoprecipitation and double immunodiffusion. Major histocompatibility complex class II alleles were iden… Show more

“…Anti-CENP-B antibodies were identified by enzyme-linked immunosorbent assay (ELISA) and anti-CENP by IIF (performed by J-LS) (33). Anti-Th/To antibodies were determined in sera displaying a nucleolar pattern of ANA, using protein A-assisted immunoprecipitation for nucleic acid analysis from nonradiolabeled HeLa cells (performed by MK and GB at the University of Sherbrooke) (34). Anti-topo I antibodies were determined by addressable laser bead immunoassay (ALBIA) and ELISA (35)(36)(37).…”

“…In the largest published study of SSc among twin pairs (8), there was no difference in disease concordance rates between monozygotic and dizygotic twins (overall concordance rate of 4.7%), providing evidence against a genetic influence on disease occurrence. However, since that was a cross-sectional study, it is possible that the lifetime concordance for SSc in twins may increase over time.…”

“…For instance, the biggest risk factor for SSc is family history (2). On the other hand, concordance for SSc is around 5% in twins and is similar in monozygotic and dizygotic twins, whereas antinuclear antibodies are detected more frequently in the healthy monozygotic twin sibling than in the healthy dizygotic twin sibling of an SSc patient (3). In addition, most SSc susceptibility genes are HLA haplotypes and non-HLA immunerelated genes that are shared by other collagen diseases (4).…”
