Background: Carcinoma of unknown primary site (CUP) has a poor prognosis and the prognostic factors in these patients are not well established. Furthermore, there are no selection criteria for patients who should benefit from chemotherapy. Methods: The medical records of 179 CUP patients who were treated at Taipei Veterans General Hospital from 2000 to 2009 were reviewed. Factors associated with survival were determined by Kaplan-Meier analysis. Differences between the groups with and without palliative chemotherapy were analyzed. Results: Univariate analysis revealed multiple prognostic factors, including performance status, lung metastasis, number of metastatic organs, serum albumin, corrected serum calcium, lactate dehydrogenase (LDH), sodium, and cholesterol levels, palliative chemotherapy, and white blood cell and lymphocyte counts. Multivariate analysis showed that performance status < 2, serum albumin level ≥ 3.5 g/dl, corrected serum calcium level < 10.7 mg/dl, single metastatic organ, and palliative chemotherapy were independent factors of better prognosis. Patients with better performance status, higher serum albumin, and lower serum LDH levels had significantly greater benefit from palliative chemotherapy. Conclusions: Certain patients with unfavorable CUP will have better survival. Identification of patients with unfavorable CUP who could benefit from palliative chemotherapy warrants future prospective studies.

Peripheral T-cell lymphomas (PTCLs) are a heterogenous group of aggressive non-Hodgkin's lymphomas that are incurable in the majority of patients with current therapies. Outcomes associated with anthracycline-based therapies are suboptimal, but remain the standard of care for most patients, even though the benefits of this approach remain uncertain. This study retrospectively examined outcomes in a cohort of North American PTCL patients treated with both anthracycline- and nonanthracycline-containing regimens. The incorporation of anthracycline-containing regimens was associated with improved progression-free survival (PFS) and overall survival (OS). Patients treated with nonanthracycline-containing regimens were more likely to have high-risk features and were less likely to undergo high-dose therapy and stem cell transplantation. However, anthracycline use remained an independent predictor of improved PFS and OS when adjusting for these confounding variables. Anthracycline-based regimens and consolidation with high-dose therapy and autologous stem cell transplantation in appropriately selected patients remains a viable option for patients unable to participate in a clinical trial. Long-term disease-free survival is not optimal, highlighting the need for an improved understanding of disease pathogenesis, and the development of novel therapeutic strategies.

Primary mediastinal large B-cell lymphoma (PMLBCL) in Chinese patients: clinical characteristics and prognostic factors

Primary mediastinal large B-cell lymphoma (PMLBCL) is a unique clinico-pathological subtype, for which there is no optimal therapy yet. We evaluated clinical characteristics and prognostic factors in 39 consecutive, previously untreated Chinese patients with PMLBCL. The median age was 28 years (range 16-72 years). The majority of patients were stage I/II (23 cases, 59%). Bulky mediastinal mass was present in 18 cases (46%). The 3-year overall survival (OS) and progression-free survival (PFS) rates were 70 and 64%, respectively. Cox regression analysis showed that low serum albumin was an independent prognostic predictor of both OS (P = 0.002) and PFS (P = 0.001). Other prognostic factors for OS were extra-thoracic involvement (P = 0.016) and B symptoms (with borderline significance, P = 0.053). The addition of radiotherapy to chemotherapy seemed to have a favourable impact on OS (P = 0.034) and PFS (P = 0.039). The addition of rituximab to chemotherapy improved the survival trend, but added no significant benefit (OS: 57 vs. 84%, P = 0.287; PFS: 53 vs. 73%, P = 0.371). Our data additionally suggest that low serum albumin is a novel prognostic predictor for PMLBCL, and could be useful in determining treatment options in the clinic.
