Analysis of patient flow in the emergency department and the effect of an extensive reorganisation

Abstract: Objectives: To evaluate the different internal factors influencing patient flow, effectiveness, and overcrowding in the emergency department (ED), as well as the effects of ED reorganisation on these indicators. Methods: The study compared measurements at regular intervals of three hours of patient arrivals and patient flow between two comparable periods (from 10 February to 2 March) of 1999 and 2000. In between, a structural and staff reorganisation of ED was undertaken. The main reason for each patient remai… Show more

“…Miro et al increased their ED bed capacity from 25 to 41 beds and observed significant decreases in the number of patients waiting to be seen and waiting room time. 14 The absence of any effect of ED expansion on overcrowding and ambulance diversion can be explained using the conceptual model for ED overcrowding by Asplin et al They described three interdependent components: input, throughput, and output. 15 The expansion of the ED was an attempt to increase patient throughput; more patients could be evaluated at one time.…”

“…Value-stream map is a diagram that identifies all the steps in the patient pathway from order to delivery. [12] Time consumed for each step from registration to disposal was measured and documented on the value-stream map so as to quantify the amount of value-added and non-value-added time in each step. Figure 3 illustrates the important time determinants of patient fl ow.…”

“…13,14 Studies of patient flow have identified factors impacting throughput and have proposed several monitoring and remedial strategies. [15][16][17][18][19][20] Our study shows how interrelated these two areas are on a theoretical level. We demonstrate that workload and patient flow are linked via a simple input-output relationship.…”
