Introduction:The most vulnerable road users are pedestrians. This article discusses important aspects of a few selected medico-legal issues that have received relatively little attention in Sri Lanka thus far.Methodology: A cross-sectional study was conducted in which 500 patients who were admitted to the Accident Service Unit (ASU) following pedestrian-vehicle collisions between January 2018 and January 2020.Results: There were 78% (n=391) males among the victims, and the average age of the victims was 47 years. Forty-five percent (n=224) of the incidents occurred between 6 p.m. and 6 a.m., accounting for 71% of the total. Five percent (n=25) of them had encountered an incident while crossing pedestrian lines. Fourteen percent of pedestrians admitted that the accident was their fault. Eight percent (n=39) had insurance coverage, and 54% (n=272) indicated a desire to pursue compensation through litigation. Forty percent (n= 202) correctly identified the court to which they needed to go to obtain compensation. The JMO's (Judicial Medical Officer) role was not well understood, and only 04% (n=19) were aware of it precisely. Fifty six percent sustained grievous injuries and 61% of participants had been referred to other specialities for expert opinions while being treated in the Accident Service Unit (ASU). Eighty-five percent (n=427) of patients underwent surgery, and the majority stated that they were not adequately informed about the surgical procedures before surgery. Six percent (n=29) stated that they were not in pain. There was a statistically significant correlation between age and sex and the severity of the injuries sustained. The time of the accident and the fact that it occurred while crossing the road demonstrated a significant association, whereas alcohol consumption and the fact that it occurred on a pedestrian crossing did not. There was no statistically significant difference in the amount of pain experienced by males and females. When patients reached an advanced age, they became less likely to pursue compensation claims. Conclusion:The health sector was burdened with a huge number of pedestrian victims upon traffic-related injuries and the majority of victims suffered injuries either grievous or above in categorization of hurt. The knowledge of the service provided by a JMO and the court system was insufficient. Identification of peculiarities among victims helps to minimize casualties by addressing specific aspects.

Objectives: Investigating patterns among the outcomes of patients involved in motor vehicle accidents (MVAs) can provide information necessary to guide targeted interventions to improve road traffic safety. Our purpose is to identify any differences between passenger and driver injury severity and overall clinical course after MVAs. Methods: We performed a retrospective review and analysis of 3,693 patients involved in MVAs from 2016 to 2021. We divided the data into two groups, drivers and passengers, and compared the Injury Severity Score (ISS), Revised Trauma Score (RTS) on admission, days in the Intensive Care Unit (ICU), length of hospital stay (LOS), post Emergency Department (ED) disposition, discharge (DC) disposition, and signs of life on arrival (SOLA) to the ED. We compared mean ISS, New Injury Severity Score (NISS), RTS, length in ICU and LOS using a student’s T-test and SOLA, post-ED and DC disposition using Chi-square analysis. Results: We did not find any statistically significant difference in ISS, RTS, days in ICU, LOS, or SOLA between the drivers and passengers. However, we did find a statistically significant difference in the post-ED (X 2 = 113.743, p=<0.0001) and DC disposition (X 2 =41.172, p=<0.0001) of drivers and passengers. After the ED and DC, more passengers were transferred to a higher level of care than expected, while the inverse was true for drivers. The number of drivers discharged to Skilled Nursing Facilities (SNFs) was also higher than expected, further contributing to the observed difference in DC disposition. Conclusion: Our study found no statistically significant difference between driver and passenger injury severity, length of hospital stay, days in ICU, and SOLA after an MVA. The clinical courses of the two groups were found to be significantly different based on post-ED and DC disposition data. We identified limitations, such as a relatively small sample size and insufficient data on specific car seat locations for passengers, underscoring the need for a more nuanced exploration. Future research must broaden its scope to encompass diverse crash scenarios, vehicle design and safety technologies, seat belt dynamics, and age- and gender-specific vulnerabilities.

Introduction Acute medical problems may occur more frequently during Ramadan fasting (RF). We aimed to provide a narrative overview of the global literature on medical emergencies during RF. Methods This is a nonsystematic review of the international literature from one major medical online database (PubMed, National Institutes of Health, United States). The relevant literature was narrated in a concise thematic account. Results There is a variable impact in the burden and time distribution of emergency services and hospitalization during RF that may require readjustment of resource allocation. Studies of the risk of accidents and injuries may be increased overall or at specified times, around Iftar time. A classical impact of emergencies has been the risk of worsening peptic ulcer disease. RF impacts diabetic emergencies such as severe hypoglycemia, hyperglycemia, and diabetic ketoacidosis, particularly in type 1 diabetes and poorly controlled patients. Glucocorticoid replacement therapy may represent a challenge to patients and physicians that require education and dose adjustment. Acute neurological conditions of interest include the “first day of Ramadan headache,” epilepsy, and strokes. Several studies evaluated the risk of the acute coronary syndrome and heart failure with inconsistent findings. RF may impact the renal and urological systems through stone disease, renal colic, and acute kidney. The impact on hematological conditions was mostly focused on the safety of anticoagulant therapy during Ramadan. Conclusion The review addresses the emergency medical encounters of the fasting patient during Ramadan to allow a holistic and ethnically sensitive approach to medical care under circumstances where decisions have to be taken with no delay.
