Analysis of Bead Sizes for MR Capsules Labeled for Sprinkle

Abstract: Abstract. The bead sizes used in approved modified release capsules labeled for sprinkling on food was investigated to generate bead size guidelines for generic products labeled for sprinkling. The conclusions from a survey of FDA databases were corroborated with experimental data obtained by measuring the bead sizes of several reference-listed drugs on the market labeled for administration by sprinkling on food. The experimental data show that majority of the marketed products were found to have bead sizes of… Show more

“…Several formulation features of DR/ER-MPH contribute to its consistent MPH release rates despite altered evening food intake. Tiny microbeads (<1 mm in diameter), such as DR/ER-MPH, are rapidly emptied from the stomach (Davis et al 1986) and, as such, expected to be minimally affected by food-induced effects on gastric emptying (Nagavelli et al 2010). Based on the properties of the DR and ER layers, initial dissolution and absorption of MPH are not dependent on any single factor, such as pH or variations in gastrointestinal transit (Childress et al 2018), both factors that can be affected by food.…”

“…However, such hydrocortisone suspensions are not licensed, the hydrocortisone content may be inconsistent leading to treatment failures [30], and may contain sucrose, which can have adverse effects on teeth with long-term use [31]. Food is chewed to <2 mm [32]; therefore, sprinkling beads of smaller size onto food should not cause problems swallowing. Furthermore, sprinkling of medication may have advantages in improving adherence and facilitate caregiving of patients with swallowing difficulties.…”

“…They report that this size should ensure adequate mouthfeel and reduce the risk of inadvertent chewing although it is not stated whether this is based on adults or a paediatric population. The multiparticulate sizes used in approved modified release capsules labelled for sprinkling on food was reported; the majority of products had multiparticulate sizes of <1.5 mm …”

“…The multiparticulate sizes used in approved modified release capsules labelled for sprinkling on food was reported; the majority of products had multiparticulate sizes of <1.5 mm. [57] The administration may also impact upon the mouthfeel, for example products administered directly into the mouth are likely to feel more gritty than those mixed with soft food before administration. Lopez et al [58] assessed the mouthfeel of spherical cellulose multiparticulates dispersed in liquids in young adults using a VAS from very smooth to very gritty.…”
