Analysis of a diagnostic strategy for patients with suspected tumors of unknown origin.

Abstract: The limited diagnostic evaluation used in this study identified patients with treatable malignancies and increased the survival duration of a population of suspected UPT patients. Primary malignancies with the best survival can be diagnosed through careful pathologic review and focused evaluations for breast and ovarian cancer in women.

“…Multiple empirical chemotherapy regimens have been used for CUP patients, but there are few randomised data to support a specific regimen (Briasoulis et al, 2005). The median survival in randomised studies is approximately 7 months (Abbruzzese et al, 1995), which is both poor and significantly less than the expected survival for patients with breast and bowel malignancy following standard therapy. These data suggest that little further benefit can be gained from the use of empirical regimen, perhaps because of the heterogeneity of tumour phenotype represented in the true CUP population.…”

“…Extensive diagnostic procedures cause discomfort for the patient, require time and cause delay of treatment. In addition, they are often fruitless (Hainsworth and Greco, 1993;Abbruzzese et al, 1995;Schapira and Jarrett, 1995).…”

“…For the minority of tumours that have been identified in the last two decades as being potentially sensitive to chemotherapy (van der Gaast et al, 1990;Pavlidis et al, 1992;Abbruzzese et al, 1995;Lenzi et al, 1997;Greco and Hainsworth, 2001b), the diagnostic procedures will delay these patients from receiving effective treatment.…”

“…However, in spite of clinical, radiographic, and routine histologic studies, the primary site remains uncertain in approximately 3 -15% of cases (Abbruzzese et al, 1995;Hammar, 1998). A significant proportion of these cases are adenocarcinomas, and they present a challenge for the pathologist and the oncologist alike.…”
