The Government's effort improves the health status of the Indonesian people by establishing a Jaminan Kesehatan Nasional (JKN). JKN’s program was formed by Badan Penyelenggara Jaminan Sosial Kesehatan (BPJS Kesehatan). Payment of health costs by BPJS uses the Indonesian-Case Based Group (INA-CBG's) especially for advanced health care services. INA-CBG's is payment to the main health center for a predetermined service package according to the disease diagnosis category. This makes hospitals strive to continue providing quality health services to JKN patients while still controlling costs. The aim of this research is analyze the difference between the outpatient BPJS patient rates and INA-CBG's rates at the Genteng Regional General Hospital in 2021. The research method is a retrospective observational study with simple random sampling. The total sample was 400 obtained during the period January to December 2021. The results of the research showed that the hospital rates was IDR 105,995,056 and the INA-CBG's rates was IDR 89,484,300. It was known that there were three largest cost components including drugs (drugs and chronic drugs) was IDR 51,878,574 (48.70%); patient care costs was IDR 33,980,600 (24.65%); and consultation fee was IDR. 10,400,000 (9.40%). The three largest components of hospital rates included minor chronic disease procedures and dialysis procedures. The conclusion was hospital rates are greater than INA-CBG's rates.

The province that ranks fourth with the highest incidence of hypertension in Indonesia is the Special Region of Yogyakarta (DIY). Hypertension therapy is carried out in a not short period, so it becomes an economic burden for people with hypertension and the government. This study aims to determine the relationship between the length of hospitalization and direct medical costs—an analytical observational research method with retrospective data collection and data from class III inpatient hypertensive medical records. The calculation of costs is viewed from a payer's perspective—descriptive and analytical data analysis to determine the relationship between the length of hospitalization and direct medical costs. The study's results of 41 patients met the criteria; 53.65% were women aged 22-59 years (53.65%). The most widely used antihypertensive drug was the combination of amlodipine + candesartan (26.82%). The average direct medical costs for hypertensive patients were IDR 7,039,515. The highest direct medical cost component is the cost of other drugs and medical equipment. The component of direct medical costs is the lowest cost of antihypertensive drugs. The average length of hospitalization for hypertensive patients is 6.14 days. There was a significant relationship (p=0.000) between the length of hospitalization and direct medical costs. The conclusion of this study is a significant relationship (p = 0.000) between the length of hospitalization and direct medical costs

Hypertension as a cardiovascular disease ranks first as a disease with the highest expenditure on health insurance costs. In the financing system implemented by the Indonesian Social Security Program, hospitals as health service providers no longer break down bills based on details of services provided, but only by submitting a diagnosis code that comes out in the form of Indonesia Case-Based Groups (INA-CBGs). The INA-CBGs code for mild hypertension with a severity level of 1 is I-4-17-1. The purpose of this study was to determine the implementation of INA-CBGs rates by comparing the cost difference between the INA-CBG rates and the actual hospital rates for inpatients with hypertension code I-4-17-1 with the parameters studied in the form of gender, age, and length of stay, comorbidities, and drug usage. The analytic observational method with a cross-sectional design was used. Data were hospital financial records and patients medical record documents (inpatient hypertension patients) coded I-4-17-I with the following classes; first class, second class, and third class treatment rooms from October 2016 to July 2017 at Panembahan Senopati Hospital. Data were tested for normality with the Kolmogorov-Smirnov test. One Way ANOVA test and t-test were used to analyze the data in determining the difference in costs between the INA-CBGs rates and the actual hospital rates and in determining the factors that influenced the difference in costs. The results showed that 53 patients met the inclusion criteria. The total cost incurred by the hospital for hospitalized hypertensive patients was Rp. 140,963,105, while the total claim rate for INA-CBGs was Rp. 109,960,000. Gender, length of stay, comorbidities, and drug usage were contributed to the difference in cost, while age had no significant effect. It can be concluded that hospitals need to evaluate the implementation of INA-CBGs rates to minimize financial losses.
