Hemorrhoids is one of the most common gastrointestinal diseases. There are several therapeutic options associated with some complications. Therefore, researchers look for traditional medicines as a potential resource for introduction of new natural drugs. The current study reports an evidence-based review of herbal remedies for hemorrhoids in traditional Persian medicine. A comprehensive survey about hemorrhoids on the most important manuscripts of traditional Persian medicine was done. Then, scientific data banks were searched for possible related properties of each herb in the conventional medicine. We reported some historical aspects of traditional Persian medicine view on classification, examination, and predisposing factors of hemorrhoids. In addition, we have reported 105 medicinal plants belonging to 51 families. More than half of the reported herbs exhibited anti-inflammatory and analgesic effects. Although lack of human studies regarding the mentioned herbs is noted, positive results from experimental findings can be considered for new drug discovery supported by traditional and medieval experiences.

Problem statement: The aqueous leaf extract of Bryophyllum pinnatum has been reported to possess antihypertensive, antiinflammatory, antimicrobial, antidiabetic and antiulcer properties among others. Being readily available and easy to cultivate, the use of the plant in herbal medicine has been on the increase. Although there are few toxicological reports on the extract, these have not been sufficiently extensive. We therefore designed this study in order to evaluate more extensively the acute and sub-acute toxicity of the extract in rats. Approach: Oral and intraperitoneal (i.p) LD 50 experiments were conducted on Sprague-Dawley rats. Other rats were given daily doses of 2 g kg −1 body weight (p.o) × 35 days at the end of which kidneys, hearts, spleen and blood/sera were obtained for weight, haematological and biochemical analyses. Results: While there was no death at a maximum acute dose of 5 g kg −1 body weight by the oral route, the intraperitoneal LD 50 was 1.8 g kg −1 body weight. Subacute treatment did not significantly alter animal weights, organ-to-body weight ratios, fluid intake, hematological indices and the levels of AST, ALP and albumin. ALT level was significantly reduced (p<0.03) in the treated group. Total bilirubin and conjugated bilirubin levels were not significantly altered in the treated group. Conclusion: The results point to the possible safety of the aqueous extract although more toxicological studies are needed to confirm this.

Medicinal plants still remain a source of medical care in developing countries, in the absence of a modern medical system. The use of herbal medicine is deeply rooted in our culture, because Algeria is renowned for the richness of its medicinal flora which includes hundreds of plant species. Ecballium elaterium (L.) A. Rich., the species reported in this study is traditionally used in Algeria for a wide range of illnesses. The aim of this study was to evaluate the potential toxicity of it. Many herbalists (two hundred) in different regions in Northern Algeria (Tébessa, Khenchela, Oum El Bouaghi, Constantine, Skikda were questioned to recognize the toxicological concern of E. elaterium in Algerian traditional medicine in these regions.
