Anaesthesia for spinal surgery in adults

Abstract: The spectrum of spinal surgery in adult life is considerable. Anaesthesia for major spinal surgery, such as spinal stabilization following trauma or neoplastic disease, or for correction of scoliosis, presents a number of challenges. The type of patients who would have been declined surgery 20 yr ago for medical reasons, are now being offered extensive procedures. They commonly have preoperative co-morbid conditions such as serious cardiovascular and respiratory impairment. Airway management may be difficult. … Show more

“…14 Patients with severe cervical cord injuries are more prone to hemodynamic and cardiac abnormalities within the first 1-2 weeks after injury. 1,2 Lehmann et al 1 found hypotension in 68% of the studied patients with SCI with complete cervical cord injury and no hypotension in either incomplete cervical or thoracolumbar cord injuries.…”

“…25 At that time, reflex hyperactivity begins manifesting as muscle spasms and elevation of blood pressure. 14,26 Furthermore, Ko and coworkers 27,28 specified that the time of return of deep tendon reflexes after SCI is within the first couple of weeks after injury with the delayed plantar reflex being the first followed by the bulbocavernous and cremasteric reflex. However, no established connection between spinal shock and neurogenic shock does exist.…”

“…The exact mechanism of potentiation of vasoconstriction by clonidine remains unclear. Although Tanaka and Nishikawa attribute this vasoconstrictive action of clonidine to postjunctional alpha-1 adrenoceptor agonism (29), Talke et al suggest that clonidine acts on the alpha-2b subtype of alpha-2 adrenoreceptors in peripheral vascular smooth muscle to cause vasoconstriction (32).…”

“…Factors other than blood pressure, postulated to affect intra-operative blood loss include intra-abdominal pressure (related to prone positioning), the number of spinal segments being operated on, body weight, the pathological entity of the disease necessitating surgery (spine surgery due to tumoral lesions is associated with more bleeding), and surgeon's experience (32). In our study, all patients were operated due to traumatic fractures of the spine on 3 to 4 spinal segment levels and by the same surgical team.…”

“…[4] Scoliosis should be treated at the proper time because especially idiopathic scoliosis has a rapidly progress and may be fatal following the possible complications. [5] Some other types of scoliosis may also have dangerous medical conditions. [6] It is clear that scoliosis has a multifactorial etiology and it is generally presented in idiopathic type.…”
