An Unusual Osteochondral Articular Surface Fracture of Second Metatarsal Head: A Case Report

Abstract: Osteochondral fracture involves the articular cartilage and it creates bone or cartilage fragments that can range in various size and depth. An isolated osteochondral fracture of metatarsal head is extremely rare. The treatment of these fractures is controversial. Authors present a case of an osteochondral fracture of second metatarsal head that was treated by open reduction and internal fixation with a headless compression screw.

“…An isolated shear fracture of a metatarsal head in the lesser toes is a rare injury, and few cases have been reported. 1,3,9 All previously reported fracture locations (the second, third, and fourth metatarsal heads) were present in our case series. Fractures usually occur at the second and fourth metatarsal, 1,6,9 which is consistent with the data in our study.…”

“…Various fixation techniques and implants have been utilized for treatment of these fractures. 1,4,6,7,9 We stabilized the fractures by 1 or 2 headless compression screws, or 1 screw and a K-wire, according to the size of the dorsal fragment. In addition to ORIF, dorsal wedge osteotomy and osteochondral autograft transplantation have been reported as treatment options.…”

“…The failure of blood supply can eventually lead to avascular necrosis. Tanaka et al ,2 Temiz et al 14 and Liddle et al 11 performed open reduction of the fracture fragment. In the current case, we also performed open reduction of the fracture with careful handling of neurovascular structures.…”

“…Several operative techniques and implant types have been used for such second MT head fractures. Temiz et al 14 used 18 mm headless compression screw for fixation. Liddle et al 11 used two, 2.0 mm screws.…”
